Bear Mountain Healthcare is looking for a dedicated and compassionate MDS Nurse Coordinator (RN) to join our team. As an MDS Nurse Coordinator, you will play a vital role in providing high-quality care and support to our resident family. Responsibilities:
- Assessments and Care Plans: As the MDS Nurse Coordinator, you will be responsible for completing assessments, Minimum Data Set (MDS), and care plans for all assigned residents. This includes conducting comprehensive evaluations of residents' needs and developing individualized care plans to ensure the best possible outcomes.
- Monitoring and Compliance: You will monitor the completion of MDS assessments by other disciplines within the timeframes mandated by regulatory guidelines. If any assessments are incomplete or untimely, you will promptly notify the supervisor for resolution.
- MDS/RAPs/Triggers: Ensuring accurate and timely completion of the MDS/RAPs/Triggers sheet for assigned residents will be among your primary duties. This involves meticulous attention to detail and adherence to established guidelines.
- Care Plan Conferences: You will conduct care plan conferences for assigned residents, involving other members of the care team and the residents' families to ensure comprehensive care planning and effective communication.
- Care Plan Updates: Regularly maintaining and updating care plans and assessments for assigned residents is crucial. These updates should occur at least on a quarterly basis, and you should add or delete issues as necessary to address changing needs.
- Incident and Accident Review: You will review all resident incidents and accidents, along with the 24-hour report daily, to ensure that care plans for assigned residents reflect current and evolving needs accurately.
- Customer Service: As a representative of Bear Mountain Healthcare, you will uphold and promote the facility and company's standards for superior customer service. Your positive, courteous, and helpful behavior will contribute to a welcoming and caring environment for all internal and external customers.
Minimum Job Skills Requirements:
- RN Licensure: Candidates must hold a current RN license in the state of Massachusetts.
- Vaccination: Compliance with MA regulations is essential, and candidates must be fully vaccinated.
- PDPM Experience: Prior experience in long-term care/skilled nursing facilities and expertise with the Patient-Driven Payment Model (PDPM) is a must.
