Partnership Solutions is Hiring an Enrollment Specialist Near Bronx, NY

Overview:

The Enrollment Specialist is the initial point of contact for people inquiring about and seeking care management information and support from Care Design New York. The Enrollment Specialist ensures a suitable entry into services and a smooth hand-off to care management staff. Responsibilities include but
are not limited to assisting with eligibility, gathering documentation, interfacing with OPWDD's Front Door staff, and acting as a liaison with LGUs, school districts, hospitals, and other service providers who may be seeking to refer individuals for services, and communication with regional CM staff to ensure a smooth transition into services.

Responsibilities:
  • Acts as an initial point of contact for individuals, families, advocates, and professionals who are reaching out to inquire about accessing Care Management and other supports available through OPWDD.
  • Assesses and determines the need for OPWDD eligibility. For those who do not yet have OPWDD eligibility, help support and navigate in obtaining OPWDD eligibility.
  • Refer and assist members in identifying/scheduling/obtaining needed evaluations.
  • Collects relevant information and assessments to support people in eligibility and enrollment and submits to OPWDD for review.
  • Review and complete psychosocial updates for potential members.
  • Works with OPWDD on completing the LCED for individuals wishing to enroll in either Health Home Care Management or HCBS Basic Plan Support.
  • Works closely with OPWDD Front Door staff to ensure expedient service entry.
  • Remains current in changing OPWDD policies and procedures to ensure that CDNY remains responsive in supporting people's entry into support.
  • Coordinate interim support services with OPWDD, service providers, and internal CM resources when needed for identified high-risk members.
  • Collaborate with CDNY departments to determine individuals' Medicaid status and support needs.
  • Works closely with regional leadership to ensure a smooth transition to an appropriate Care Manager once OPWDD approves.
  • Remain knowledgeable about community resources.
  • Represents CDNY at regional information fairs and events as necessary.
  • The remote enrollment position is subject to change depending on organizational needs.
Qualifications:
  • Bachelors Degree and 2 years of experience working with people with IDD (unless grandfathered).
  • Proficiency in the use of internet-based applications, Chrome, and Outlook. Must be comfortable navigating computer-based documentation and electronic timekeeping and training.
  • Ability to learn all required computer applications.
  • Proficiency with health-related computer applications is also required.
  • Must also be able to travel and adhere to CDNYs travel policies.
  • Reliable transportation and a valid driver's license may be required depending on location
