Position Summary: The Home Health and Hospice Medical Social Worker provides supportive services to patients and their families in their homes. The ideal candidate will address the social, emotional, and psychological needs of patients, helping them navigate the challenges of illness, disability, and end-of-life care. This role involves collaborating with interdisciplinary teams to ensure comprehensive care and support.

Key Responsibilities:

Conduct psychosocial assessments of patients and families to identify needs and resources.
Develop and implement individualized care plans in collaboration with the healthcare team.
Provide counseling and support to patients and their families to cope with illness, disability, and end-of-life issues.
Educate patients and families about available community resources and assist with accessing these services.
Advocate for patients’ rights and coordinate care with other healthcare providers and community resources.
Assist with discharge planning and continuity of care.
Maintain accurate and timely documentation in accordance with agency policies and procedures.
Participate in interdisciplinary team meetings to discuss patient care plans and progress.
Provide crisis intervention and support as needed.

Qualifications:

Master’s degree in Social Work (MSW) from an accredited institution.
Current state licensure as a Medical Social Worker.
Minimum of 1-2 years of experience in a healthcare setting, preferably in home health or hospice care.
Strong understanding of medical terminology, chronic illness, and end-of-life care.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and as part of a multidisciplinary team.
Valid driver’s license and reliable transportation for home visits.
Proficiency in electronic health records (EHR) and basic computer skills.
