As our full-time On Call Telehealth nurse practitioner, you will be working remotely (must be licensed and Live in Maryland) to patients in skilled nursing facilities (SNF) and Assisted Living Facilities in MD to treat geriatric and rehabilitation patients for their PM&R and pain management needs. You will be responsible for telehealth visits and maintaining a strong relationship with the client. Excellent clinical and interpersonal skills are required as well as the desire to be a part of a team that functions at the highest level. Your duties will include collaboration with administration, nursing, therapy, and families as well as preventative care. The ideal candidate will always put the patient and facilities first.
