The Health Record Technician is responsible for assembly and chart analysis, reception duties, birth and death certificate completion, and electronic record processes including scanning, indexing, and maintenance of folder structures.
Duties/Responsibilities:
• Collects all discharged patient charts daily.
• Prepares documents for scanning, scans, and indexes each document with the correct patient and document ID into the electronic record.
• Analyzes medical records of discharged patients for completeness and accuracy, and enters deficiencies as required for physician completion.
• Monitors the timely completion of records and notifies physicians, removing any completed deficiencies.
• Responsible for suspension of physicians with delinquent records per established procedures.
• Conducts medical record reviews in accordance with Joint Commission regulations.
• Receives, sorts, and delivers all departmental mail to appropriate personnel, and processes outgoing mail to referring facilities/providers.
• Obtains information pertinent to the child/parent from the obstetrical and newborn records to prepare and complete the birth certificate.
• Notifies physicians upon receipt of death certificates to maintain compliance for timely death certificate completion.
• Maintains the Master Patient Index.
• Responsible for coordinating the release of medical information to all requesters. Responsible for maintaining security and confidentiality regulations for record access and review.
• Assists patients with registration or questions with the patient portal.
Education:
High school graduate or equivalent required
Experience:
None required. One (1) year of clerical experience preferred
