Department:######## Community Pharmacy Reports To:######### Pharmacist in Charge ######### # SUMMARY The Community Pharmacist is responsible for providing dispensing, compounding, and counseling services in accordance with the standards, laws, regulations and policies of the profession, applicable regulating bodies, and the Health System. The Community Pharmacist will also participate in and help coordinate clinical pharmacy programs. # Essential Duties and Responsibilities#include the following (others may be assigned) # Professional Responsibilities Maintains a high standard of practice, using the American Society of Health-System Pharmacists Standards of Practice, and the American Pharmacists Association Code of Ethics to support the Pharmacy#s activities# Maintains professional competence through participation in continuing education programs.# Maintains, encourages, and supports involvement and contact with professional peer organizations Helps ensure all services offered by the Pharmacy are delivered with superior customer services. Helps supervise the efforts of personnel in his/her area of responsibility Helps ensure employees in his/her area of responsibility are disciplined according to Health System policy, as needed. Helps ensure all the pharmacy#s recordkeeping systems are in place, are complete and accurate to comply with all applicable laws, regulations and policies. Acts as resource person for medical and Health System staff on Pharmacy related issues and provides input to Health System policies, procedures and decisions. Helps ensure the Pharmacy is prepared for State Board of Pharmacy audits at any time. Assists in training new pharmacy employees and students Helps ensure appropriate disposal of waste pharmaceuticals in his/her area of responsibility. # Clinical Responsibilities Provides all clinical pharmacy services including, but not limited to: professional pharmaceutical consulting, disease state management, medication therapy management, and immunizations as allowed. Instructs, providers, Health System staff, students, the public, etc., on the specifics of medications Communicates to providers and other staff any discrepancies, clarifications, clinical or medication safety concerns, etc. Reviews prescriptions for accuracy and appropriateness Considers age-related factors in care, treatment, education, and evaluation of patients ranging from neonates, pediatrics, adolescents, adults and geriatrics.# Applies knowledge of age-related factors appropriately in accordance with professional standards and departmental policy and procedure Compounds, dispenses and labels chemicals and pharmaceutical preparations according to prescription orders Supervises compounding of medications as necessary Monitors and helps to maintain appropriate inventory of medications. Provides medication education, upon request, to physicians, other staff, patients, and the communities we serve.# Helps develop clinical activities for patients; provide continuity in clinical patient monitoring and drug use evaluations as necessary.# Identifies and documents any unethical or unorthodox usage of prescriptions or pharmaceuticals. Identifies and collects data on possible Adverse Drug Reactions and Medication Occurrences.# Works with Medication Safety committee, Medical Staff and the Pharmacy Director to target system changes and establish corrective pathways. # Computer Entry and Filling Accurately input prescription information for transmission to insurance companies, thereby decreasing risk of audit; understand proper DAW use and documentation Resolve rejected claims by speaking with coworkers, providers or third-party insurance companies to help best serve our patients Accurately count, measure or mix prescription medications Accurately present final preparation of prescription medication in appropriate container, applying proper labeling (including auxiliary labels) and maintaining pharmaceutical elegance Communicate medication profiles (including medication summaries for tax purposes) verbally or on paperto requesting individual as allowable by privacy laws Access the Minnesota Prescription Monitoring Program with suspected drug abuse/diversion behaviors (i.e. patient using multiple providers, early refills, using cash instead of insurance) and follow-up with appropriate course of action # Health System Responsibilities Performs according to FirstLight Health System Mission and Vision.# Demonstrates Health System values in daily work Participates in staff meetings; attends and participates in other meetings as necessary. Maintains education in regards to safety, fire, blood and body substances as required by accreditation and regulating bodies. Supports the policies and procedures of the Health System and the standards of the Joint Commission as required for full accreditation. Verbalizes Right-to-Know, specific to Oncology storage, preparation and disposal. D. from an accredited collegeof Pharmacy program.
