VPI is Hiring a Mental Health Navigator Near Appleton, WI

JOB SUMMARY:

The Mental Health Navigator serves Calumet, Outagamie, and Winnebago counties. The intent is to decrease the number of calls to law enforcement by getting individuals connected directly to the mental health care system. The Navigator receives referrals and connects these individuals to community counseling and psychiatry services. The Navigator helps coordinate transportation and address other barriers to connecting to community mental health care. To ensure individuals referred to the program are well connected to community mental health services, the Navigator follows individuals through three completed appointments with the community provider.

ESSENTIAL FUNCTIONS:

1. Professionally interfacing with law enforcement and community agencies to receive referrals for services.

2. Documenting all contacts with referrals in a web-based case management system.

3. Taking phone calls, text messages, and emails from referrals and referral sources to collect necessary information.

4. Partnering with local mental health service providers and community agencies to refer individuals for services.

5. Working with and providing support to individuals in need of mental health care.

6. Scheduling mental health appointments for referred individuals.

7. The Navigator also provides support to ORC programming, including planning and facilitating groups and providing one-to-one support to ORC members.

8. Working with marketing to send out newsletters on the program.

9. Collecting data on referrals, outcomes, and other relevant metrics.

10. Working collaboratively with the Mental Health Navigator Partners – N.E.W. Mental Health Connection, Medical College of Wisconsin, Ascension, United Way Fox Cities, and other stakeholders

11. Plan and facilitate ORC groups and provide one-to-one support to ORC members.

12. Performs other duties as assigned.

PHYSICAL DEMANDS:

Functions are considered light to moderate according to the US Bureau of Labor Statistics. Work performed includes the ability to move freely (standing, stooping, walking, bending, reaching, pushing, and pulling), and lift to a maximum of twenty-five (25) pounds without assistance.

WORK ENVIRONMENT:

Normally works in a climate-controlled office environment. Frequent sitting with movement throughout office spaces. Use of computers throughout work. Frequent use of keyboard with repetitive motion of hands, wrist, and fingers.

JOB QUALIFICATIONS:

1. Bachelor’s degree in psychology, social work, human services, or related field.

2.Minimum 1-year experience working with individuals with mental illness and/or substance use disorders.

3. Ability to efficiently utilize technology, including a web-based case management system, Microsoft Office, and related equipment.

4. Ability to adhere to professional ethics and standards.

5.Communicate effectively with clients and family members, community agencies and service providers, psychiatrists, physicians, other medical professionals, human services personnel, school personnel, and others verbally and in writing.

6. Work well in a team environment displaying a supportive and positive attitude.

7.Demonstrated common sense, judgment, and communication skills and the ability to maintain confidentiality.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
