Position Summary:
Under the direction and supervision of the New Beginnings InHome Services Manager and Staffing Coordinator is responsible for providing andassisting participants with activities of daily living, restorative andsupportive care necessary to preservation of the home environment. Assists in monitoring the health status ofand providing a safe environment for participants. Functions as a member of themultidisciplinary team.
Home Health Aide responsibilities include:
Providing health care services in patients residences
Performing domestic and household tasks
Transporting and accompanying patients to doctors office orto hospital
Responsibilities
·Provide health care services in patients residences
·Perform domestic and household tasks
·Transport and accompany patients to doctors office or tohospital
·Administer simple prescribed medications
·Assist with clients personal care activities
·Monitor patients (vital signs, temperature, respiration, etc)and report on their condition
·Maintain patients care records and document provided services
·Assist patients with mobility and physicaltherapies/exercises
·Instruct and counsel patients and families on diet andexercise
·Collect routine specimens
·Provide companionship and basic emotional or psychologicalsupport
Requirements and skills
·Proven working experience as Home Health Aide
·Ability to monitor vital signs and to collect specimens
·Familiarity with basic nutrition and personal hygienestandards
·Proficiency in English
·Nursing and healthcare administration skills
·Caring and compassionate personality
·Current CPR certificate
·First Aid training
·Valid current driver’s license and auto insurance andreliable transportation
·High school degree
·Must be 18 years or older
Physical Requirements and Working Demands
·Requires the ability to lift/carry up to 65 pounds and pushor pull objects exceeding 150 pounds, using appropriate body mechanics. Requires intermittent driving thoughout day,between participants/client’s homes.
·Requires full range of body motion including handling andlifting patients, manual and finger dexterity and eye-hand coordination. Requires standing, walking, pushing, bendingkneeling and reaching at arm’s length and overhead in community for prolongedperiods of time. Must be able totransfer participants needing total assist.
·Must be able to make home visits to participants who live innon-handicapped accessible dwellings.
·May be exposed to a risk of bodily injury through contactwith moving instrumentation, toxic substances, medicinal preparations, bodilyfluids, cleaning supplies, communicable diseases and any other conditionscommon in a home environment. Subject tounpleasant odors and living conditions of participants. Maybe subjected to adverse driving conditionsincluding wet, icy, slushy and snow packed roads.
