Role Description

This is a full-time on-site role as an MDS Coordinator at Lourdes Senior Community in Waterford, MI. The MDS Coordinator will be responsible for care planning, creating and maintaining care plans, and managing the Minimum Data Set (MDS) assessment process. Additionally, the coordinator will collaborate with the healthcare team to ensure accurate and comprehensive documentation of resident care needs.

Qualifications

• Licensed Practical Nurse (LPN) with a valid state license
• Experience in care planning and creating care plans
• Knowledge and understanding of the Minimum Data Set (MDS) assessment process
• Strong background in geriatric nursing
