The Keswick Community Health Associate will support the delivery of all service line programs including Communities for Life (CFL℠), Housing Upgrades to Benefit Seniors (HUBS), Home & Healthy (HH), and Wise & Well Education. Under the direction of the Senior Director, this associate will be familiar with daily operations and responsibilities including but not limited to: service navigation, membership customer service, class preparation and facilitation of wellness education. This role will support clients through the coordination of wrap around services and connection to community resources that enable the community member to live well at home for as long as possible.
