Human Service Agency is Hiring a Case Manager - Brookings Near Brookings, SD

Description of Position:
The mission of New Horizons is to support people to achieve self-reliance and to exercise control over their lives. The Case Manager is responsible to guide and provide individualized supports to people. This position is responsible to assist each person supported to achieve personal outcomes that reflect their dreams, hopes, and desires.
Position Details:
Full Time: 40 hours per week (Monday - Friday)
Shift: Days
Job Knowledge and Comprehension:
A. Understands and demonstrates a commitment to supporting people in achieving their hopes, dreams, and wishes (personal outcomes).
B. Serves as a member of the Personal Support Team, which designs the personal support plan for each individual to ensure the most effective methods of behavior modification or treatment.
C. Ensure people's health, safety, and welfare are not compromised through unreasonable or unnecessary risk.
D. Coordinates needed supports for people supported.
E. Ensure people have the needed financial resources by utilizing all available resources and making sure that all necessary paperwork is completed.
F. Help people manage funds and manage representative payee funds in a responsible manner.
G. Acts as an advocate for people making sure that people's rights are not violated and ensuring needed supports are secured.
H. Knowledgeable of assessment and other processes used to discover people's hopes, dreams, wishes, and preferences. Uses the information that is discovered from the assessments and other processes to develop an Individual Support Plan (ISP) for that person.
I. Monitors crisis situations, discusses the incident with authorized staff and people supported. Documents information from pertinent conversations with others. Adjusts supports as recommended and needed to help the person remain safe and healthy.
J. Knowledgeable of and complies with regulations for reporting to the Division and other Agencies.
K. Serves as a liaison for people, their families, guardians, and other agencies.
L. Provides leadership and supervision to all Direct Support Professionals to ensure compliance with accreditation standards and agency policies and procedures.
M. Provides services, supervision, and accountability to services and activities to ensure people are protected from physical, emotional, sexual, or psychological abuse or mistreatment.
N. Ensures that minutes are taken and recorded in the New Horizons system and submitted to all team members in a timely manner.
O. Other duties as assigned consistent with Exempt functions.
Physical Requirements
The employee is frequently required to sit and work extended periods of time at a computer monitor. Occasionally, this position may require lifting that exceeds 50 pounds during medical emergency situations or behavior support. Job requires bending, stooping, climbing stairs, pushing/pulling, and driving a vehicle.
Bachelors degree in behavioral science is preferred, or some combination of education and experience in Human Services, Social Services, or Health Care.
Two years experience working with people that have intellectual developmental disabilities.
Knowledge of computers.
Valid drivers license and safe driving record.
Successful completion of a criminal background check.
The Board of Directors of the Human Service Agency shall have the prerogative to hire staff whose qualifications may vary from the above. This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. It is the responsibility of the employee to read, understand, and comply with HSA policies and procedures that apply to protected information and security. Questions about EPHI and HSA Policies should be directed to the HSA Security Officer. Services provided by HSA will be welcoming, accessible, integrated, and responsive to the complex needs of people with co-occurring disorders/conditions.
This is a 24/7 facility. Hours are as needed and assigned.
