Responsibilities:
- Provide direct support to clients in their homes or community settings
- Assist with personal care tasks such as bathing, grooming, and dressing
- Aid in mobility and transfers, including lifting when necessary
- Drive clients to appointments, errands, and social outings
- Monitor and record vital signs as needed
- Offer emotional support and companionship to clients
- Collaborate with healthcare professionals such as nurses to ensure holistic care
- Perform light housekeeping tasks to maintain a clean and safe environment

Skills:
- Experience in direct patient care or social work preferred
- Ability to lift and assist with heavy objects or individuals
- Proficient in providing hospice care and end-of-life support
- Familiarity with taking vital signs and basic medical terminology
- Valid driver's license with a clean driving record
- Strong communication and interpersonal skills
- Empathetic and patient-centered approach to caregiving
