Jon Title: Respite Provider
Location: De Pere, Wi.

Benefits for Provider:

• $14.13/hour for shifts under 8 hours
• Flexible Schedule! Mainly looking for afterschool care during weekdays.(proposed schedule: M-F 3p.m.-6p.m.)
• Occasional weekends, if provider is available.

Responsibilities of Provider:

• Assist with picking up kiddo from school.
• Assist with bringing kiddo to and from after-school activities when needed.
• Assist with homework when needed.
• Ensure care and companionship while kiddo is under your supervision.
• Hang out and have fun! This kiddo enjoys: playing with his cars and trucks, being outside, and playing football.

Qualifications of Provider:

• Valid Driver's License (needed for transportation)
• Must pass background check.
• Must be ok with cats and dogs in-home.

*If hired, Consumer Direct Care Network (CDCN) will not be your legal employer on record. If hired by a Member or their Managing Party, your legal employer on record would be the Member. CDCN’s role is to assist the Member or Managing Party in recruiting a Direct Care Worker. The Member or Managing Party has the sole authority to hire or dismiss any potential Direct Care Worker.

Pay: $14.13 per hour

Expected hours: No more than 24 per week
