Job Summary:

The Social Worker is responsible for managing multiple clients and their individual needs. They will meet with clients and their families to discuss recent successes or setbacks to their situations, developing treatment plans for clients with mental illnesses or clients recovering from traumatic events and frequently communicating with clients to ensure their physical and mental health.

Duties:

• Evaluate clients and develop care plans based on their needs, limitations, and desires.
• Address client concerns and goals while maintaining consistent communication with the client and their team.
• Encourage clients to maintain a healthy lifestyle, talk about their feelings and pursue their hobbies while learning to live with their illnesses and past traumas.
• Work directly and compassionately with clients as well as building a rapport with both clients and their families.
• Must be able to remain patient and calm during stressful situations.
• Provide therapeutic support to all clients and their families as needed.
• Actively participate in management-related activities to ensure the success of the organization.
• Maintain up-to-date records and other documentation to demonstrate outcomes and progress toward strategic goals.

Requirements:
- Bachelor's or Master's degree in Social Work with valid LMSW license.
- Ability to relate to and effectively communicate a diverse set of solutions
- Ability to make client assessments and develop and implement care plans
- Ability to provide after-hours service as needed
-Ability to express ideas clearly and concisely, orally and in writing
- Must be computer literate and proficient skills in the use of email, Word and Excel
- Self-directed, highly motivated worker who can work independently
- Must possess a valid Iowa State Driver's license and maintain a favorable driving record according to the DOT
