Assisted Living on Long Island is looking for a dynamic and energetic candidate to join our high performance team in the Case management Department. We "Redefine Care with Love". Job responsibilities include but not limited too communicating with families, apply for public assistance (ALP/Medicaid/SSI), document resident activities on an ongoing basis, and working with the other departments to ensure proper care and documentation. This position reports to the administrator. 2 years experience in long term care a plus with Case Management experience a plus.
