Cadence Care Network is looking for a Young Adult Case Manager to work in the Bridges Program, which provides extended services for youth aging out of the foster care system until the age of 21. The Bridges Program serves youth throughout Toledo, Ohio (Lucas, Wood, and contiguous counties).

Duties can include:

• Assist young adults with the completions of all required Bridges documentation in order to maintain eligibility.
• Support participation by using a client centered approach, respecting their preferences.
• Ensure required documents are maintained in SACWIS, including contact notes and case information.
• Appear in court with young adults, as needed.
• Support Bridges participants’ participation in focus groups.
• Demonstrate cultural competency and a desire to serve young adults, understanding child welfare, LGBTQ identities, and parenting.
• Facilitate living arrangement payments, food, clothing, and other living arrangements, case management, monthly contacts, and service referrals.
• Secure documentation from young adults that can be used to verify eligibility.
• Assist young adults in developing and achieving goals for independent living and help them learn to utilize services and supports to help them meet their needs.
• Ensure young adults have Medicaid or other health insurance, and assist the young adults in receiving the medical, vision, dental, and behavioral health services they need.
• Coordination and assistance in crisis management and stabilization as needed.

Requirements:

• Bachelor's degree
• Experience working with transition aged youth.
• Be willing to work a flexible schedule with a “whatever it takes approach” to serving this vulnerable population.
