The PSH Case Manager provides outreach, engagement and advocacy for homeless individuals and families participants in scattered site permanent supportive housing. The Case manager will perform assessment intake, case management and other supportive services to assist persons eligible for housing services and will assist participants in meeting their personal life goals, such as increasing their financial stability, improving their wellness and reaching in greater self-determination. This person should reflect the values of our Faith Statement and mission purpose of Restoring Hope and Changing Lives.
While this position is not a hybrid-remote position, employee may go offsite to conduct house visits, inspections and emergency case management when case management cannot be performed on site.
