About the Position:

The primary function of the Population Health Program Coordinatoris to support the coordination, performance, and evaluation of assigned services and value-based programs. This role will work in collaboration with multidisciplinary teams internal and external to Riverwood. This individual will serve to enhance population health by supporting the integration of patient care services with community resources and ensuring key deliverables are achieved for our value-based programs. This role works collaboratively with others to restore health and promote wellness by coordinating care of patients of all ages, gender, economic and social status, and backgrounds.

What we are looking for:

• Bachelor’s degree in Business Administration, Healthcare Administration, Nursing or related field required. Two or more years of experience or an equivalent combination of education and experience sufficient to successfully perform the essential responsibilities and duties of the job.
• Minimum of year experience in project/program management preferred.
• 2-3 years of experience with population health, value-based contracts, and/or ACO experience strongly preferred.
• Clinical experience or operational experience in a clinic setting preferred.
