Job Title/Position: Intake Manager
Reports To: Executive Director/Administrator
JOB DESCRIPTION SUMMARY
Responsible for managing all aspects of the patient intake process for all programs. Responsible for the day-to-day oversight of the Intake Department including Intake Coordinators. Establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies for all locations.
2. Ensures compliance with all state, federal, and ACHC referral/intake regulatory requirements.
3. Establishes the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
4. Establishes and maintains positive working relationships with current and potential referral sources.
5. Reviews patient medical diagnosis, prognosis, medications, procedures, and clinical course.
6. Provides appropriate training to all team members to ensure alignment with compliance with eligibility standards.
7. Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
8. Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
9. Negotiates service pricing with insurance Case Managers and other payers within established financial and credit parameters.
10. Builds and monitors community and customer perceptions of Calstro Hospice Inc. as a high-quality provider of services.
11. Gathers, collates, and reports referral statistics including key customer referral trends.
12. Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
13. Maintains comprehensive working knowledge of Calstro Hospice Inc. contractual relationships and ensures that patients are admitted according to contract provisions.
14. Participates in quality assessment performance improvement teams and activities.
15. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Calstro Hospice Inc.
16. Collaborates with other members of the management team to enable a smooth transition for patients and families enrolling in the hospice program.

POSITION QUALIFICATIONS
1. License Vocational nurse with current license to practice in California.
2. At least three (3) to five (5) years’ experience verifying insurance, intake coordination.
3. One (1) year of management experience preferred.
4. Ability to work with an interdisciplinary team representing other functional areas that directly or indirectly impact client intake/admission.
5. Ability to travel up to 25%. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
6. Prior use of Electronic Medical Records (EMR), preferred.
7. Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practices.
8. Demonstrates good communications, negotiation, and public relations skills.
9. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
Job Type: Full-time
Pay: $90,000.00 - $95,000.00 per year
