*Job Details Overview: *
As a *Registered Nurse* with Personal-Touch Home Care, you'll be integral in planning, organizing, and delivering home care services. We're seeking experienced nurses with a passion for community health education and engagement. Utilizing community resources, you'll tailor services to meet the unique needs of individuals and families within their homes and communities.
*Responsibilities: *
* Conduct comprehensive assessments of patients and families to determine home care needs.
* Provide thorough physical assessments and medical histories.
* Continuously reassess patient needs and adjust care plans accordingly.
* Collaborate with physicians to develop personalized care plans.
* Involve patients and families in the planning process.
* Implement preventive and rehabilitative nursing procedures.
* Administer medications and treatments as prescribed.
* Offer counseling and health education to patients and families.
* Identify discharge planning needs and implement plans prior to discharge.
* Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.
*Qualifications: *
* Current licensure in the State of Virginia.
* CPR certification and a valid driver's license.
* Preferably, a minimum of one year of recent acute or long-term care experience.
* Bachelor's degree preferred, with at least one year of experience in home care.
* While management experience is not required, you will be responsible for supervising Home Health Aides and Personal Care Aides.
* Strong observation, verbal, and written communication skills.
* Excellent problem-solving abilities and basic math skills.
