Midland Care is hiring a Community Resource Specialist for our KCK PACE Center.
In this role, you will play a crucial role in connecting individuals and families with vital social services and resources in the community. Your work will directly impact the well-being of our participants by ensuring they have access to the support they need to live their best lives.
Midland Care's Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
For Midland Care PACE participants, PACE means a team of experienced doctors, nurses and other health care professionals getting to know the participant and working with him or her to design a plan to aid in achieving health goals.
Employment Status: Full-Time|Monday - Friday, 8:00 a.m. to 5:00 p.m.
Pay range beginning at $21/hr. pay based on experience and requirements of role.

Qualifications
Experience in a health care setting, community resources, and experience with adults. Good communication and advanced computer skills required. High degree of proficiency with MS Office products required. Must be able to multi-task and be a team player. Must possess valid Kansas Driver's License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by Midland policy. Experience with behavioral health and/or mental health treatment models preferred.
