Community Workforce Solutions Inc. is searching for High School Transitions Facilitators for students with IEP's and 504's in Wake County!

Under the supervision of the Transition Instructor, this position is responsible for supporting the instruction of direct services to students (ages 14-22) with disabilities. This includes understanding and being able to communicate individually or in groups and to facilitate learning of topics related to transitioning to adulthood.

High School Transitions Facilitator duties may include:

• Supporting a caseload of 3-4 high schools across wake county.
• Creating lesson materials, data entry, billing and classroom support.

Minimum Qualifications/Requirements:

Associates Degree in Psychology, Special Education, Rehabilitation or a related human services field preferred. Looking for a candidate with experience working with students with developmental disabilities. Equivalent experience will be considered in lieu of education. Must have valid NC Driver’s License and driving record that adheres to Agency standards. Must be able to work in various schools across the county. Must pass background check and drug screening.

Physical Requirements:

The position is considered sedentary to light, requiring 50% sedentary and 50% mobility. Travel between multiple schools within the county. Lifting less than 10 pounds. Requires visual observation and the ability to communicate with a variety of individuals.
