Interim Housing Coordinator – Full-Time Position

Position Summary:
Provides ongoing assessment, case management, care coordination and monitoring of consumers needing Interim Housing. Coordinates with the transitional emergency housing program and provides case management services to individuals while admitted to Interim Housing.
Essential Functions:
· Serve as the primary point of contact for all guests, providing them with information and assistance related to their housing.
· Assess guest’s needs to help support identified housing and/or humans service goals.
· Navigate in, out and through various structures in a residential setting which may or may not be handicapped accessible.
· Assess, develop, and implement service plan goals that are short-term resulting from Interim Housing intake.
· Provide assistance and encouragement in accessing appropriate services to maintain living, learning and working in the community.
· Complete Room searches as needed in the event of concerns.
· Ensure Chores/cleaning is completed within Interim Housing.
· Complete ordering of items for Interim Housing, record keeping of cleaning supplies/furnishings/hygiene and other guest supplies.
· Coordinate with Clinton Lycoming Joinder Housing Representative and attend appropriate community outreach meetings associated with Interim Housing.
· Coordinate with Crisis Intervention Services for emergent mental health needs of guests.
· Complete required training for safe medication management, NARCAN and other identified specialist training to benefit guests.
Qualifications:
Associate’s degree in human services or related field required. One year of case management or housing or human services related field.
Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances will be required. Individuals who have accessed human services are encouraged to apply. EOE/ADA

Salary Range: $18/hr
