Job SummaryThis position will coordinate the activities of the Community Health Worker (CHW) Training Hub for Linn, Benton, and Lincoln counties, including facilitating CHW training workshops, community agency Traditional Health Worker (THW) inventory and readiness assessment, policy planning and development, data gathering, and grant reporting and monitoring.

Works closely with other agencies and organizations. Leads efforts to develop the CHW Training Hub, including sustainability planning.

• Minimum QualificationsThe following minimum qualifications are required for this position : Associate’s degree or two-year technical certificate from an accredited college or university in a field related to area of assignment;
• AND 2 years of experience in a Community Health Worker / Health Navigation position or related experience in a healthcare clinic / team-based primary care setting;

ANDMust have or be able to obtain Community Health Worker Certification with the Oregon Health Authority within the first 6 months of employment.

An equivalent combination of education and experience may be accepted. Please note candidates must have at least 6 months of actual work experience to be considered as equivalent.
