Responsibilities:

• Conduct comprehensive assessments of patients' psychosocial needs and develop individualized care plans.
• Provide counseling and emotional support to patients and their families to help them cope with illness, hospitalization, and related stressors.
• Assist patients and families in understanding and accessing community resources and services.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated care.
• Advocate for patients' rights and needs within the healthcare system.
• Facilitate discharge planning and continuity of care by coordinating with community agencies and resources.
• Document patient interactions and care plans accurately and timely, maintaining confidentiality of patient information.
• Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
• Provide crisis intervention and support during emergencies.

Medical Social Worker Qualifications:

• Master's degree in Social Work (MSW) from an accredited program.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of CA.
• 1-2 years of clinical experience as a Medical Social Worker, preferably in a healthcare setting.
• BLS/CPR certification required.
• Valid CA Driver’s license and reliable transportation.

Job Type: Part-time

Pay: $42,000.00 - $65,000.00 per year
