HAPPI Health is seeking a highly motivated medical professional for a busy medical clinic with 3 locations. HAPPI is a Federally Qualified Health Center - Look-alike (FQHC-LAL) and is certified as a Patient-Centered Medical Home (PCMH). A positive attitude, ability to multi-task and a desire to care for the under-served is a must. The clinic uses Athena for its electronic medical record system - experience in Athena is preferred. Bilingual in Spanish is a plus.
Job Title: Family Nurse Practitioner
Reports to: Chief Medical Officer
Education/Training: Advanced practice registered nurses with nurse practitioner certification.
Basic Function: Ability to work autonomously and in collaboration with other healthcare professionals to deliver family-focused care. Provide a wide range of healthcare services that revolve around the family unit; from health promotion and disease prevention to direct care and counseling across the lifespan.
Responsibilities:
· Developing treatment plans for acute and chronic diseases
· Educating and guiding patients on disease prevention and healthy lifestyle habits
· Understanding the changes in health promotion throughout the aging process
· Conducting exams
· Performing diagnostic tests and screening evaluations
· Managing overall patient care regarding lifestyle and development issues
· Emphasizing preventative care and disease management
· Prescribing medications
· Completing documentation and billing of provided services in a timely manner
· Participate in core measure initiatives, meaningful use, etc.
· Provide a 90 day written notice of separation
· Consistently work a regular schedule (must work at least 15 full shifts {7 hours counts as a full shift} a year to maintain collaboration) and notify in writing about disruptions or significant changes to schedule to arrange appropriate coverage (ideally arrange coverage for your shifts)
Nurse Practitioner must be able to rotate between locations (pediatric primary care, family primary care, and urgent care) for assignments. Mileage reimbursement is available for travel between locations if needed. Some weekends required. Rotating after-hours call required.
Physical Requirements:
Hearing: Adequate to perform job duties in person and over the telephone/ able to use stethoscope and respond to verbal questions.
Vision: Visual acuity adequate to perform job duties, including visual examination of patient, reading information from written and printed sources, as well as computer screens.
Other: Requires frequent lifting and carrying items weighing up to 30 lbs. unassisted, including assisting patients when required. Adequate physical ability to include sufficient manual dexterity to perform requisite health care service, including injections. Requires frequent bending, reaching, repetitive hand movements, standing for up to 4 hours without a break, walking, squatting and sitting, with some heavy lifting, pushing, and pulling exerted regularly throughout a regular work shift. Additionally, requires some exposure to communicable diseases, bodily fluids, medicinal preparations and other conditions common to a clinic environment.
