Description
Job Summary:
Evaluates individual needs of clients for Vision Rehabilitation and provides consultation and instruction with follow up of progress and reports.
Duties/Responsibilities:
Evaluates individual needs of clients and provides instruction in Vision Rehabilitation Therapy.
  • Contact within 2 weeks of assignment.
  • Accurate projection of hours.
  • Accurate identification of client needs, skill level, and goals.
  • Client improvement in VRT skills as indicated by observation, client, customers, and follow-up form.
  • Collaborates with other professionals as needed to ensure comprehensive benefit to clients and identifies other needs and refers as appropriate.
  • Adequate preparation/planning for lessons, individual or group.
  • Response to calls are handled promptly and accurately.
Maintains information on client service, including evaluation, progress, and final reports.
  • Timeliness of reports (within one week after completion of evaluations and within two weeks after completion of instruction).
  • Timeliness of data for statistical and billing purposes.
  • Specific breakdown and explanation of recommendations in evaluation.
  • Written documentation accurately describes client skill level and progress and gives a clear, descriptive explanation of teaching content.
  • Completes client inventory and billing procedures.
Initiate and actively participate in professional activities to help meet program or agency needs.
  • Actively participate in program planning, program development and identification of unmet needs.
  • Fully prepared for talks/presentations/projects; projects meet goals and are completed within timeframe.
  • Collaborate with other staff as needed.
  • Apply and share professional knowledge in a variety of ways.
  • Assist in implementation of the unit's and department's annual plan.
Provides consultation, information, and on-going service to maintain client independence.
  • Information is obtained to handle requests appropriately.
  • Information on equipment is provided.
  • Referrals to other services occur as indicated.
  • Initial response to request occurs within 2 days.
  • Consultations regarding diabetic management are scheduled in a timely manner.
Qualifications
Education and Experience:
  • Master's degree in Vision Rehabilitation Therapy; dual certification in O&M a plus.
  • Previous training and experience working in vision rehabilitation therapy preferred.
  • ACVREP Certified Vision Rehabilitation Therapist (CVRT), or commitment to obtain certification.
Required Skills/Abilities:
  • Excellent oral and written communication skills.
  • Proficient with Microsoft Office, including Word, Excel, and Outlook.
  • Ability to travel throughout the service area in southwestern Ohio and Northern Kentucky.
