JOB DESCRIPTION
Title of Position: Case Coordinator Homeless Services
Location: Gateways Hospital & Mental Health Center
Exempt/Non-Exempt: Non-Exempt
Union/ Non-Union: Union
Supervisor: Program Director at Homeless Services
SUMMARY OF POSITION:
The Case Coordinator position is a FTE position. The Case Coordinator assists consumers with locating, obtaining housing, maintaining housing, and developing housing resources, and is responsible for providing case management services, principally supportive of improvement in life skills and ADLs, to adults with serious mental illness. The position requires budget adherence, submission of reports, chart files, group activities and facilitation of oversight entities (including ACOF, HACoLA, LAHSA, DMH, Shelter Care Partnership.)

Minimum Experience Required
One (1) year experience mental health, counseling, or substance abuse treatment.
Academic training in Behavioral Sciences
Desired Experience
Two (2) years’ experience in mental health, counseling, or substance abuse treatment. Experience in homeless outreach, housing placement, LAHSA program or homeless services.
EDUCATION & CERTIFICATES
Desired Education
Bachelor’s degree in Behavioral Sciences or related area with two (2) years experience
Minimum Certification /
Licensure Required
California Class C Driver’s License
Driving record acceptable for coverage by Gateways Hospital insurance carrier
Fire and Safety Training*
CPR Certification*
Management Of Aggressive Behavior*(M.O.A.B)
REQUIREMENTS
**Training needs to be completed within 30 days from the date of hire on a designated date provided by on-boarding. Staff need to be certified at the commensurate level for their role to perform their job duties (staff will be taken off the schedule if certification is not achieved).
