Role Description
This is a full-time on-site role for a Hospice Social Worker, located in Columbus, OH. The Hospice Social Worker will provide emotional support, counseling, and resources to patients and families facing end-of-life challenges. They will collaborate with the interdisciplinary care team to ensure patients' psychosocial needs are met and assist families in navigating grief and bereavement.
Qualifications
- Experience in Medical Social Work and general Social Work practices
- Knowledge and skills in End-of-Life Care and providing Grief Counseling
- Familiarity with Discharge Planning processes
- Strong interpersonal communication and active listening skills
- Ability to work effectively within an interdisciplinary team
- Master's degree in Social Work (MSW) and licensure required
