Licensed Vocational Nurse (LVN) – Skilled Nursing (SNF)
Location: Modoc Medical Center – Alturas, CA
Contract: 13 Weeks
Shift: Nights | 36 Guaranteed Hours/Week
Job Summary:
Modoc Medical Center is seeking a dedicated and compassionate LVN for the Skilled Nursing Facility (SNF). This role works under the supervision of the DON and nursing leadership to deliver quality, individualized care to residents. Ideal candidates are dependable, adaptable, and committed to providing excellent patient care.
Key Responsibilities:
- Perform comprehensive patient assessments and report abnormal findings to Charge Nurse/MD
- Administer medications per facility policy and maintain accurate documentation
- Provide wound care, treatments, and assist with direct patient care as needed
- Educate patients and families on care plans
- Respond to emergency situations per protocol
- Maintain infection control and safety standards
- Collaborate with interdisciplinary staff to ensure continuity of care
- Assist with orientation of new staff when needed
- Maintain confidentiality and uphold facility mission and values
Requirements:
- Active California LVN license
- Current BLS Certification
- Minimum 1 year LVN experience (SNF preferred)
- Ability to walk, lift, bend, and perform essential physical duties
- Strong communication, assessment, and organizational skills
- Must wear Caribbean Blue scrubs (facility requirement)
