Job Title: LPN
Location: Ridgewood, NY
Pay Rate: $34-40/hr
Position Overview: We have an immediate opening for a LPN's who specialize in phlebotomy.
Key Competencies:
- Each day, you will drive to patients’ homes to provide care and monitoring
- While at a patient’s home, engage in assessments and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing the traditional healthcare system
- Throughout the treatment process, maintain patient medical records and documentation consistent with state regulations, and policy through our HealthPoint and Athena EMR system?
- You will be practicing under direction with synchronous telemedicine support
Duties and Responsibilities:
- Act as a conduit for the medical treatment of the Health Plan Partnerships patient population
- Navigate conversations related to management of health, well-being, medication management, and goals of care
- Provide supervised, directed treatment of common medical diagnoses
- Adhere to clinical and safety standards, protocols, and performance metrics
- Provide care with cultural sensitivity and compassion
- Lead your practice with a patient-centered approach
