Description

Zepf Center has been serving the Lucas County community for over 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.


We are currently seeking a full-time Substance Use Disorder (SUD) Case Manager who assumes responsibilities for ongoing wraparound integrated care coordination services provided to an assigned caseload of adult consumers in the SUD and/or combined SUD/MH treatment programs. Client-centered services are delivered in the office, consumer homes, and community setting to consumers who are experiencing substance use disorders and/or co-occurring disorders. This position is responsible for providing support, advocacy, case management, treatment planning, reassurance, care coordination, linkage, referrals, and counseling to consumers in our outpatient programs.


Essential duties of this position include but are not limited to:

  • Provides therapeutic client-centered interventions including, but not limited to, service coordination, integrated care, linkage, referrals, follow up, healthcare management, resource access, advocacy, individual/group/family counseling, urinalysis, transition planning, wellness programming, and participation in/facilitation of care coordination/team meetings.
  • Monitors clients to identify potential relapse, new treatment needs, change in level of care, and effectiveness of the IAP.
  • Facilitates referrals and linkages to appropriate internal and external resources based on assessed needs.
  • Maintains clinical responsibility for those assigned clients who have co-occurring mental health and substance use disorders.
  • In conjunction with the client, family and/or significant other, develops and updates a strength-based Individualized Action Plan (IAP) for each client based on the completion of a case management assessment/IAP review at least every 90 days.
  • Responsible for accurate and timely documentation which includes but is not limited to: program admission and discharge forms, activity/progress notes, case management assessments/IAP reviews, IAPs, Level of Care forms, DPS logs, resource access applications, releases of information, monthly reports, online productivity tracker forms, incident report forms, regular email usage and replies, and any other forms necessary to document services.
  • Provides assistance in gaining access to essential community resources.
  • Provide ongoing support of consumers’ individual recovery processes.
  • Provide quality chemical dependency treatment according to program and agency requirements.
  • Use appropriate motivational techniques to engage consumers in successfully completing their treatment.
  • Maintain a healthy environment that will promote the recovery process.
  • Provide basic chemical dependency education, counseling and case management, that consistently directs consumers toward achieving full recovery (i.e., disease of addiction, blackouts, withdrawal, effects on self and others, triggers, relapse process and prevention).

Requirements

  • High school diploma, with 3 verifiable years working within the behavioral health field OR Bachelor’s degree in a related field.
  • Working knowledge of, and experience working with, persons who have mental health diagnoses as classified by the DSM 5
  • Knowledge regarding chemical dependency knowledge, including the addictive process, withdrawal, and the recovery process.
  • Must have current/active Chemical Dependency Counselor Assistant (CDCA) certification in good standing through the Ohio Chemical Dependency Professionals Board (OCDPB) at hire.
