Position Summary:

Under the direction of the Director of Women in Jeopardy, the Health and Wellness Coordinator will provide comprehensive health and wellness resources and services to participants at the YWCA. This encompasses a broad spectrum of support, including mental health counseling, assistance with insurance navigation, access to medical care and addressing a variety of other health-related needs to ensure holistic well-being. In compliance with the American Rescue Plan Act (ARPA) grant, the coordinator will develop creative solutions for underserved populations, establish community partnerships to support domestic violence survivors, and work directly with adults and children in shelter, KRH and the community to assess needs and connect them with appropriate services.  

 


Essential Job functions:

Client Services:

  • Complete client needs assessments following referrals from supportive services staff
  • Provide clients with ongoing health resources and follow-up care, leveraging existing partnerships to navigate the healthcare system and connect them to resources tailored to their long-term well-being needs.
  • Manage current client scheduling and transportation services, ensuring seamless coordination with other departments.
  • Manage and coordinate the logistics of support groups, therapy appointments, and on-site mobile clinic visits.
  • Facilitate effective communication between healthcare providers and clients while adhering to confidentiality protocols.
  • Discover innovative approaches to provide health and wellness resources for marginalized communities.
  • Streamline payment processes for specific client needs.


Grant Compliance and Reporting:

  • Submit comprehensive quarterly reports to DHHS
  • Track client information and services through Efforts to Outcome (ETO).
  • Ensure adherence to current ARPA grant requirements, projects and deadlines.
  • Stay informed about the latest COVID-19 protocols
  • Efficiently manage and track ARPA budget


Community Partnerships:

  • Ensure the continuation and cultivation of existing community partnerships with healthcare providers, behavioral health professionals and any other relevant stakeholder, while also fostering the development of new collaborations as necessary.
  • Develop new partnerships to serve clients in the healthcare setting.
  • Organize mobile medical clinics to visit the YWCA and serve a wide range of medical needs.
  • Ensure the ongoing distribution of pamphlets aimed at educating community partners about domestic violence.

 

Additional Duties and Responsibilities:

  • Uphold and act in accordance with the organization's mission, vision, values and strategic direction.
  • Demonstrate commitment to understanding and using Sanctuary Model principles, language, and tools in daily work.
  • Model YWCA community standards and encourage program participants and other staff to follow them.
  • Understand and follow all established organizational and program related policies, procedures and protocols.
  • Keep all necessary records to ensure that work is conducted in accordance with agency, contractual, and legal requirements. Records are expected to be complete, accurate, and timely.
  • Satisfactorily perform all duties outlined in this job description
  • Maintain all certifications and licensing related to role, program licensing, and grant standards including CPR, First Aid, Food Handlers Permit, LCSW/CSW/SSW, etc.
  • Attend all required staff meetings, individual and/or group supervision, agency-wide staff and other training, and any other meetings and activities as requested.

Ideal candidates will have the following:


  1. A strong foundation in direct victim services, covering violence against women, sexual violence and the provision of tangible support to those affected. Additionally, a feminist perspective would underscore the importance of understanding gender and race equity, addressing systemic inequalities through advocacy and empowerment.
  2. Knowledge and experience in non-profit operations, programs, and protocols, with a demonstrated track record in working with grants and cultivating partnerships.
  3. Welcoming & inclusive personality
  4. Understanding of equity, anti-oppression and trauma-informed philosophies.
  5. Proficient in Health and Wellness program management with experience in navigating healthcare systems and utilizing available resources

 

Qualifications:

  • Experience with working in a residential setting.
  • Knowledge and skills in effective communications, crisis intervention, women's issues, human behavior and community resources.

  • Skilled in problem-solving and adept at navigating intricate social systems.
  • Commitment to program excellence and accountability.
  • Collaborative team player who works effectively with colleagues and staff across programs, agency.
  • Excellent computer skills including proficiency with MS Office and other database(s).
  • Fluency in English (speaking and writing) required, with fluency in Spanish helpful.
  • Ability to work assigned hours regularly and have some flexibility related to coverage of a 24/7 shelter.
  • Ability to work respectfully with people of diverse ages, races, ethnicities, nationalities, socio-economic levels, beliefs and lifestyles.
  • Must be able to pass BCI, meet I-9 requirements, and, if driving on agency business, have a valid Utah driver’s license, automobile insurance and motor vehicle record.

 

Work is generally performed in a safe and comfortable office environment. While performing the duties of this job, the employee is regularly required to: sit for long periods of time; use computer to enter data; use telephone to converse with customers and employees; walk and stand for long periods of time; perform repetitive wrist, hand and/or finger movement. Travel to attend conferences, training, outreach and other events occurs routinely.
