YWCA Lancaster is looking for a full-time Case Manager in our Kepler Hall Residence program, who is an integral part of our team.
The Kepler Hall Case Manager provides comprehensive resident services designed to promote self-sufficiency, stability, and community engagement. This role includes conducting assessments, coordinating resources, delivering direct support, and fostering positive relationships with residents and community partners.
What your day looks like:
Resident Services & Case Management
- Conduct initial needs assessments and collaborate with residents to create and follow individualized service plans that encourage independence. This includes conducting biweekly quality meetings with emergency shelter families and quarterly quality visits with permanent residents.
- Provide direct support in areas such as independent living skills, employment case management, budgeting assistance, and rental payment compliance.
- Connect residents with community resources related to employment, health, education, transportation, housing, and other essential services.
- Develop and maintain a resource library of services and opportunities available to residents.
- Coordinate on-site and off-site programming to engage residents and increase awareness of available resources.
- Gather resident feedback and maintain regular communication to monitor progress and address concerns.
Crisis Intervention & Compliance
- Follow established crisis intervention protocols and provide support during emergencies or challenging situations.
- Address resident concerns promptly and escalate serious issues to the Residence Director.
- Report adult neglect to the Office of Aging when appropriate.
- Serve as a mandated reporter for child abuse and neglect; coordinate services with local agencies and the school district homelessness liaison.
Administrative & Professional Standards
- Obtain and maintain licensure to access Empower Lancaster: Homeless Management Information System (HMIS).
- Maintain timely and accurate case notes; track outcomes and progress toward goals. Ensure intake and departure procedures are completed accurately and in a timely manner.
- Maintain professionalism and confidentiality in all interactions.
- Build and sustain positive relationships with residents and community partners to foster collaboration and support.
Kepler Hall Programming
- Create and facilitate welcome orientations for new residents and families, including preparing materials and coordinating welcome packets.
- Engage volunteers to assist with resident services, such as preparing rooms for incoming residents and coordinating supportive activities that foster community engagement.
- Plan and facilitate monthly resident information meetings in collaboration with the Residence Director.
- Assist Director and other residence staff with regular programming to engage the resident community.
- Other duties, as assigned.
Who you are:
- Bachelor’s degree in Social Work, Psychology, or a health-related field (including mental health), Master’s degree in Social Work, Psychology, or a health-related field (including mental health) is a plus.
- Minimum of 2 years of case management experience required.
- Demonstrated commitment to a trauma-informed workplace that balances kindness, competence, and care toward self, colleagues, and populations served.
- Proficiency in Microsoft Office Suite and Windows-based environments.
- Strong written and interpersonal communication skills.
- Ability to complete YWCA Lancaster’s Sexual Assault Prevention and Counseling Training Course, plus 12 hours of additional related training, upon hire.
- Ability to work independently and collaboratively as part of a team.
- Strong organizational skills and attention to detail.
- Flexibility to work occasional nights and weekends.
- Ability to travel within the local community as needed.
- Assist Director and other residence staff with regular programming to engage the resident community.
- Experience working with unhoused or under-housed populations strongly preferred.
- Knowledge of local services, community partners, and resources preferred.
- Ability to converse fluently in English and Spanish preferred.
- Other duties, as assigned.
