Transitional Supportive Housing Case Worker

Position Overview

We are seeking a compassionate Transitional Supportive Housing Case Worker to support women and children living in a communal supportive housing program. Residents may be experiencing housing instability, financial hardship, or other life challenges, and some may also be survivors of domestic violence.

This role helps families stabilize after crisis, build independent living skills, and transition to permanent housing. Using a person-centered and trauma-informed approach, the Case Worker provides case management, housing support, and resource coordination while helping maintain a safe and respectful communal living environment.

Key ResponsibilitiesCase Management & Support

• Conduct resident assessments and develop individualized housing and service plans.
• Meet regularly with residents to monitor progress and provide ongoing support.
• Encourage self-sufficiency and long-term housing stability.

Housing Stabilization

• Assist residents with housing searches, applications, and lease agreements.
• Educate residents on tenant rights and responsibilities.
• Provide housing retention and eviction prevention support.
• Support residents transitioning to independent housing.

Advocacy & Resource Coordination

• Connect residents with community resources, public benefits, healthcare, and employment services.
• Provide advocacy and support for residents facing personal challenges, including those impacted by domestic violence.
• Assist with safety planning and crisis intervention when needed.

Child & Family Support

• Support the well-being of children in the program.
• Connect families with childcare, education, and counseling services.
• Encourage healthy routines and positive parenting practices.

Communal Living Support

• Promote a safe and respectful shared living environment.
• Facilitate communication and conflict resolution among residents.
• Encourage community engagement and cooperation.

Life Skills Development

• Provide coaching in budgeting, employment readiness, and daily living skills.
• Support residents in building confidence and independence.

Documentation & Compliance

• Maintain accurate case notes and program documentation.
• Track progress toward housing and personal goals.
• Follow program policies and safety guidelines.

Qualifications

• Associate’s or Bachelor’s degree in Human Services, Social Work, Family Studies, or a related field
• Minimum of 1 year experience in case management, supportive housing, or social services preferred.
• Knowledge of person-centered and trauma-informed care.
• Ability to work with women and children facing housing instability.
• Strong communication, organization, and problem-solving skills.

Job Type: Full-time (35 hours per week)

Pay: $18.50 - $20.50 per hour

Reports to: Director of Housing Services and Community Impact
