POSITION SUMMARY

The Community Support Worker provides structured, community-based support designed to assist individuals and families in improving functioning, stability, independence, recovery, resiliency, self-advocacy, and successful participation in the community.

The CSW assists clients with implementing authorized service-plan goals, developing practical skills, addressing barriers, navigating community resources, coordinating services, communicating with providers, and strengthening natural and formal support systems.

The CSW does not independently function as a psychotherapist, diagnostician, medical provider, or licensed clinician unless the employee separately holds the appropriate professional license and YuMi formally assigns duties under that license.

ESSENTIAL DUTIES

Client Engagement

The CSW shall:

  • establish professional rapport;
  • use active listening;
  • communicate respectfully and clearly;
  • identify client concerns;
  • identify barriers to functioning;
  • reinforce strengths;
  • promote client participation;
  • support self-determination;
  • encourage self-advocacy; and
  • maintain professional boundaries.

SUPPORT-PLAN IMPLEMENTATION

The CSW shall assist clients with goals and interventions contained within the applicable service plan.

Activities may include:

  • identifying barriers;
  • breaking goals into achievable steps;
  • reinforcing progress;
  • teaching practical skills;
  • reviewing progress;
  • identifying setbacks;
  • coordinating resources; and
  • communicating relevant concerns to the treatment team.

The CSW shall not independently alter a clinical treatment plan unless authorized to do so by applicable scope and supervision.

COMMUNITY RESOURCE NAVIGATION

The CSW may assist clients in identifying and accessing:

  • housing;
  • food;
  • transportation;
  • healthcare;
  • behavioral-health services;
  • educational resources;
  • vocational resources;
  • employment services;
  • public benefits;
  • identification documents;
  • community programs;
  • legal-resource referrals;
  • disability resources;
  • family supports;
  • childcare resources;
  • social supports;
  • substance-use resources;
  • recovery supports; and
  • other services related to authorized goals.

Whenever appropriate, the CSW should teach the client how to navigate the resource independently rather than simply performing every task on the client's behalf.

SKILL DEVELOPMENT

The CSW may teach, model, reinforce, and practice:

  • communication;
  • appointment management;
  • organization;
  • time management;
  • problem-solving;
  • self-advocacy;
  • use of technology;
  • resource navigation;
  • transportation planning;
  • interpersonal skills;
  • community participation;
  • coping strategies within CSW scope;
  • employment readiness;
  • educational organization;
  • independent-living skills;
  • decision-making;
  • planning; and
  • other functional skills identified in the service plan.

SERVICE COORDINATION

The CSW may:

  • assist with referrals;
  • communicate with authorized providers;
  • coordinate community services;
  • help prepare clients for appointments;
  • assist clients in identifying transportation barriers;
  • participate in authorized treatment-team meetings;
  • follow up on referrals;
  • communicate client concerns to supervisors;
  • coordinate with community partners; and
  • support continuity of services.

DOCUMENTATION

The CSW shall complete documentation accurately and timely.

Documentation shall include applicable:

  • client name or identifier;
  • date;
  • location;
  • time or service duration;
  • service provided;
  • intervention;
  • relationship to service-plan objective;
  • client response;
  • progress;
  • barriers;
  • follow-up; and
  • CSW identification/signature.
  • Documentation shall accurately reflect what occurred.
  • Documentation may not be fabricated, inflated, backdated deceptively, or created solely for reimbursement purposes.

CRISIS RESPONSIBILITIES

The CSW shall recognize situations requiring escalation.

When a crisis or safety concern occurs, the CSW shall:

  • remain within scope;
  • follow YuMi procedures;
  • contact the appropriate supervisor or clinician;
  • contact emergency services when required;
  • maintain appropriate client safety;
  • document the event; and
  • participate in follow-up as assigned.

A CSW does not automatically become a crisis clinician by virtue of holding the CSW title.

MINIMUM QUALIFICATIONS

The employee must:

  • be at least 18 years old;
  • satisfy applicable education/experience qualifications;
  • complete YuMi-required training;
  • complete applicable CCSS training;
  • pass applicable screening requirements;
  • maintain confidentiality;
  • demonstrate professional conduct; and
  • remain eligible to perform assigned duties.

RELEVANT EDUCATION

YuMi recognizes education in fields including:

  • social work;
  • psychology;
  • counseling;
  • behavioral health;
  • human services;
  • sociology;
  • public health;
  • rehabilitation;
  • family studies;
  • education;
  • special education;
  • criminal justice;
  • community health;
  • human development;
  • substance-use studies; and
  • closely related disciplines.

REQUIRED YUMI TRAINING

The employee shall successfully complete the:

ACCL Speed Athletics Academy Community Case Worker Short Course.

The course is an internal workforce-development requirement and does not constitute professional licensure.

CCSS TRAINING

Employees assigned to provide CCSS must complete the applicable 20-hour CCSS training.

YuMi's internal goal is registration and completion as early as reasonably possible, with a target of beginning or obtaining the training within approximately 30 days of hire, subject to course availability and applicable requirements.

COMPENSATION AND CAREER LADDER

CSW TRAINEE

Base: $15.00/hour

Typical rates:

HS/GED: $15.00

Associate: $15.50

Bachelor's: $16.00

Master's: up to $17.00 with approval

Requirements:

onboarding;

Academy course underway;

CCSS registration as directed;

supervision;

satisfactory conduct.

CSW I

Base: $17.00/hour

Typical rates:

HS/GED: $17.00

Associate: $17.50

Bachelor's: $18.00

Master's: $19.00

Requires successful completion of the Academy Community Case Worker course and satisfactory performance.

CSW II

Base: $19.00/hour

Typical rates:

HS/GED: $19.00

Associate: $19.50

Bachelor's: $20.00

Master's: $21.00

Requires:

Academy course;

applicable CCSS training;

verified qualifications;

satisfactory documentation;

satisfactory supervision;

management approval.

This is YuMi's normal fully trained entry-level CSW classification.

CSW III

Base: $21.00/hour

Typical rates:

HS/GED: $21.00

Associate: $21.50

Bachelor's: $22.00

Master's: $23.00

Typically requires:

at least one year satisfactory relevant experience;

all required training;

demonstrated competency;

reliable documentation;

satisfactory annual review.

CSW IV

Base: $23.00/hour

Typical rates:

HS/GED: $23.00

Associate: $23.50

Bachelor's: $24.00

Master's: $25.00

Typically requires:

approximately two to three years relevant experience;

advanced competency;

strong documentation;

effective resource coordination;

satisfactory continuing education;

management approval.

SENIOR CSW

$25.00-$27.00/hour

Senior CSWs may:

  • mentor staff;
  • assist with training;
  • model compliant documentation;
  • assist with complex resource coordination;
  • support quality-improvement activities;
  • assist less-experienced CSWs; and
  • support management operations.

Typical placement:

$25.00: experienced Senior CSW

$26.00: advanced experience and/or relevant bachelor's degree

$27.00: advanced experience, relevant master's degree, and substantial responsibility

LEAD CSW

$27.00-$30.00/hour

Lead CSWs may:

  • coordinate workflow;
  • mentor CSWs;
  • assist with staff orientation;
  • assist with caseload monitoring;
  • identify documentation issues;
  • support scheduling;
  • coordinate resource information;
  • participate in quality improvement; and
  • communicate operational concerns to management.

Lead CSW status does not authorize clinical supervision or licensed clinical practice unless separately qualified.

Contractor Benefits

YuMi Wellness Support offers a contractor benefits program for eligible 1099 independent contractors who maintain consistent service volume with the organization.

Eligible contractors may receive:

  • 50% company contribution toward eligible health insurance premiums
  • 40% company contribution toward eligible life insurance premiums

Access to benefits after the applicable eligibility and waiting-period requirements are satisfied

Benefit eligibility is based on continued service volume and compliance with YuMi Wellness Support's contractor benefits requirements. Additional eligibility requirements and contribution terms are provided in the Independent Contractor Agreement and Contractor Benefits Policy.

Benefits are subject to applicable insurance plan requirements and do not alter the independent-contractor relationship.

Pay: $15.00 - $27.00 per hour

Expected hours: 15.0 – 40.0 per week
