Job Summary: Case Manager will be responsible for multidimensional partnerships with Employers, American Job Centers (AJCs), and Post-Secondary institutions to support participant placement. The case manager will develop work experience opportunities and targeted professional training that ensures credentialing is aligned with the needs and expectations of employers or other post-exit partners. The Case Manager will have the responsibility of processing intakes and assessment of applicants. Making referrals to service partners (for housing, nutrition, parenting, substance abuse; health care and other supports youth might need); mentoring and coaching enrollees through obstacles; setbacks, poor performance or behavioral issues. Collaborating with YBB instructors vocational educational for the purpose of ensuring that each of his/her participants have a fun, engaging, structured and impactful learning experience while participating in YBB’s vocational training programs. The Case Manager will have an active role in coordinating or the delivery of non-field site program services, life skills, morning meeting and MTO content, data entry, tracking post program outcomes of enrollees for at least 1 year and reporting findings to appropriate staff and or department.
Responsibilities:
Connecting with Partners and Outreach
1. Creates meaningful work experience and placement opportunities with partners
2. As needed check in on current students when absent or tardy
3. Develops and maintains employer engagement strategies relevant to a career pathway industry
4. Respond to inquiry emails, and voicemail pertaining to the BTE program, and conduct follow up on potential BTE and PA students
5. Cultivates working relationships with individuals and organizations to support potential training and placements
6. Support and maintain relationships with partners including but not limited to BPS Urban Science Academy (or other partner BPS), the SMART Team, Children Services of Roxbury, Father Friendly, Boston Public Health Commission, local health centers etc. to provide critical services to our youth.
Orientation, Assessments, and intake
1. Build, assemble and maintain participant folders as documentation is collected.
2. Coordinate individually or as a team, to design and deliver fun, engaging Mental Toughness orientation components including organization-wide lunches, workshops, trainings, outings, team building and certification opportunities. Help with coordinating the orientation of incoming student cohorts.
3. Ensure every student has written, specific and measurable goals in their Individual Service Strategy Plans (ISS) within the first 30 days of enrollment. Establish goals in educational, personal and/or employment areas. Use Mass CIS or other appropriate tools to identify and document aptitudes, interests, and prospective career paths as well as guide, coach, and support participants in overcoming barriers to success.
Coaching and Connecting with Partners
1. Coordinate with academic, vocational, and service instructors to ensure that each participant achieves positive outcomes – either increased literacy or numeracy, H.S.E. or N.C.C.E.R. credential attainment AND either enrollment in post-secondary institutions, advanced training programs or employment. Collaborate with vocational, academic, and service instructor(s) weekly to ensure that students receive comprehensive, integrated, and complementary services. Verify and document any gains in writing – i.e. copies of paystubs, offer letters, college transcripts, test results and/or certificates.
2. Actively engage with YBB students using positive communication, trauma-informed and developmentally appropriate strategies to motivate them to achieve goals and overcome barriers identified in their ISS Plans.
3. Oversee the referral to and delivery of legal, housing, health, social emotional or family wellness supports and other social service partner agencies. Coordinate delivery on site if necessary. Ensure youth obtain services or document attempts if unsuccessful.
Follow-up and Documentation
1. Ensure that all services, referrals, meetings with participants, their guardians, or advocates, are documented in weekly case notes kept securely in student files. Ensure that the ISS is updated bimonthly and that each file contains Mass CIS results, academic, life skill and vocational evaluations and gains, proof of certifications, credentials, and placement outcomes.
2. Maintenance and data integrity of program data systems. Based on communicated quarterly and annual deadlines, ensure data is updated accurately each month in DOL’s MIS system, the YouthBuild Database (DYB); BYSN, MA Coalition and others for timely submission of funder reports.
Qualifications:
- Spanish speaking (a plus)
- Must have driver license
- Must have reliable transportation
- Shift is 730am 330pm
- The Case Manager should have the vision, passion, and maturity to ensure students experience a safe, welcoming, and challenging academic environment that inspires them to achieve educational gains and proficiency.
- High School Diploma and a minimum of seven years related work experience with at risk, diverse and/or urban young adults OR an associate degree (or equivalent) in Human Services or other related field and five years related work experience OR a bachelor’s degree in Human Services or other related field, three years of relevant professional experience.
- 2 4 years’ experience working with young people from diverse backgrounds
- Excellent oral, written communication and computer skills
- Proficiency with Microsoft Office suite required experience with Sales Force
- Strengths in communication and interpersonal skills as demonstrated in speech, written documents, memos, contracts and emails to partners, funders, youth/guardians, and referral organizations.
- Strong administrative skills
- Ability to establish and maintain positive, effective working relationships with youth, coworkers, local agencies, community leaders and the public.
- An ability to think critically and solve problems.
- Excellent time management skills: ability to prioritize and act quickly to resolve issues.
- Must work with students using compassion, humor, maturity, coaching and behavior modification techniques.
- Must be a team player and be able to process feedback in a constructive manner
Physical Work Environment:
- The position will require walking between floors, classrooms, and offices.
- Candidates must be able to stand and sit for considerable amounts of time to meet with students, partners, and staff and to perform work on a computer.
- Must be able to bend to reach lower drawers as well as higher file cabinet drawers and participate in offsite activities on vocational weeks, service sites and field trips or tours.
- Monthly travel by T, car and on foot may be required.
