Role and Responsibilities
- Ensure interagency services are being provided to residents in accordance with the ISP and are documented and disseminated to applicable parties in a timely and professional manner.
- Coordination of all services offered to each resident in accordance with DSS and ORR policies.
- Provision of case management services as required by interdepartmental standard 22 VAC 40-151-700 for eight UIM cases.
- Director of UIM Reunification Services provides one-on-one supervision to assigned Case Manager II, Case Manager IIIs, and Lead Case Manager and reviews their cases to ensure appropriate language, timely completion of reports and tasks, and to provide ongoing training as needed.
- Participation in the development and facilitation of meetings, write the reports and monitor all service plans, including the production of ISPs and Case Reviews based on standard protocol input from the Office of Refugee Resettlement.
- Facilitate the Weekly Evaluation Team
- Participation in Evaluation Team decisions.
- Ensure planning process is oriented toward safe family reunification, preparation for return to the country of origin, transfer to a higher level of care or less restrictive setting, or transition to an alternative living arrangement once legal immigration status has been obtained.
- Consistently document the provision of services in each Unaccompanied Child (UC) case file.
- Maintain direct contact with each assigned UC in care with at least one scheduled meeting per week to discuss reunification options.
- Coordinate with entities in the field to ensure the ISP is successfully executed for each UC in care.
- Maintain constant communication with the UCs family/sponsor as well as the legal service provider and child advocate if one is in place.
- Maintain complete and updated electronic case files for the UC through FAMCARE.
- Work with Quality Assurance to ensure UC files are complete and include detailed case notes, intake, assessment, legal, education, medical, and mental health information.
- Consult on a regular basis with residential staff, clinicians, and educational staff to ensure resident is receiving optimal services and to review and discuss those areas needing attention.
- Attend conferences, meetings, and/or court hearings as needed.
- Maintain resident records, ensure compliance with Interdepartmental Regulations, and generally accepted documentation principles.
- Ensure timely documentation of all required reports.
- Ensure timely notification of appropriate personnel, parents, and agencies regarding incident reports, medical/dental emergencies, and other Significant Incident Report (SIR) events.
- Serve as primary point of contact for sponsors, attorneys, consulates and ORR representatives as needed.
- Coordinate and facilitate weekly UIM Shelter staffing with Case Coordinator, FFS, and CFS
- Send UIM Shelter ORR only Census to parties for these staffing 48 hours before the staffing.
- Coordinate and facilitate monthly UIM LTFC staffing with Case Coordinator, FFS, and CFS
- Send UIM LTFC ORR only Census to parties for these staffing 48 hours before the staffing.
- Coordinate weekly CAIR/LSP meetings and visits, whether they are virtual or in person.
- Advocate for the residents and keep staff focused on residents individual progress and goals.
- Attend required meetings and participate in formal and informal professional training.
- Pursue professional development opportunities.
- Supervision of Case Managers:
- Provide weekly supervision meetings to Case Managers
- Manage Paycom by reviewing and approving time off requests to assure there is proper coverage to the client(s).
- Assign clients/cases to Case Managers within 24 hours of placement notification to ensure even distribution of caseloads for case managers. If necessary, after reviewing the files and UC information, changes can be made in order to facilitate optimal fit based on Case Managers skillset as well as clients presenting issues or if requested by FFS.
- Oversee Case Managers with regard to file management, appropriate documentation, IEP construction, communication, and regulatory compliance.
- Provide annual evaluations for Case Managers
- Monitor and maintain Case Managers compliance with mandatory and other trainings.
- Provide Admissions with Case Assignments as well as feedback on questions they have regarding potential placements.
- Report to Associate Vice President of Programs for job duties, quarterly/annual reports, to assist with other tasks, and for supervision.
- The following Reports are due to the CFS and should be completed for UIM Shelter and UIM LTFC separately:Weekly Reporting:
- CAT 1 LOC 60 : Every Wednesday by COB
- AWOL/VD/AGE RE-De: Every Thursday by Noon
- Critical/Complex Cases: Every Friday by COB
Monthly Reporting:
- Safety and Well Being Follow Up Call Report: Every 8th of the month
- Quarterly report: (Separate reports for Shelter and LTFC) due date set by the VP of Programs
- HHS Case Management Numbers
- RPM report
- Emergency or Head Quarters Required Numbers or reports and requests for data (either sent by CFS, FFS, or PO and are not planned. Many times, a response is needed within an hour)
- LTFC Coordination of referrals. Monthly meetings with Case Coordinator may be needed to ensure responses are provided within 10 day deadline
- Facilitate LTFC monthly ET meetings.
- POC for transfer referrals. The Director of UIM Case Management Reunification Services shall review the files sent and distribute to required parties within YFT in order to provide an answer on transfer acceptance within 24 hours.
- Attendance at all training mandated by YFT and respective governing entity to include but not limited to maintaining current CPR/First Aid certification, Handle with Care certification, annual TB test/screening, and completion of biennial/triennial background checks.
- All duties and responsibilities will support meeting the structured program of care outlined in DBHDS and DSS state standards (DBHDS: 12VAC 35-46-800; DSS: 22 VAC 40-151-720) and the guidance of the Federal Office of Refugee Resettlement (ORR).
- Other duties as assigned by the CEO, Vice President of Programs, Associate Vice President of Programs
Required Trainings: First Aid/ CPR; Handle With Care: Standard Precautions; Emergency Preparedness; Shaken Baby; Mandatory Reporting; ORR training requirements and External Training to complete 40 hours of job specific related training.
Qualifications:
- Masters Degree in social work, psychology, or counseling or;
- A baccalaureate degree in social work or psychology with documented field work experience and supervision by the program director or other staff employed by the provider with the same qualifications as required by 12VAC 42-11-350 D (1. A masters degree in social work, psychology, counseling, or nursing and a combination of two years professional experience with children, in a children's residential facility and in administration or supervision; 2. A baccalaureate degree in social work, psychology, counseling or nursing and a combination of three years professional experience with children, in a children's residential facility and in administration or supervision; OR 3. A baccalaureate degree and a combination of four years of professional experience with children, in a children's residential facility and in administration or supervision.)
- Current and maintained CPR, First Aid, and Handle With Care certification.
- Current and maintained annual TB tests/screenings.
- Knowledge of Standards for Interdepartmental Regulation of Children's Residential Facilities and program services.
- Ability to communicate well with agencies, parents, and youth
- Fluent in verbal communication in English and Spanish
- Writing proficiency in English and Spanish
