Case Manager
Location: Y.E.S. Shelter in The Dalles
Schedule: Up to 40 hours per week
Reports to: Shelter Manager
Salary: $22.80 - $25.72 per hour
Purpose of the Position:
The Case Manager provides youth in the Columbia River Gorge a place of physical and emotional safety while assisting them to build positive relationships and develop their individual potentials. They build rapport with, collaborate and communicate with Y.E.S. clients, community partners and families about Individual Service Plans and goals as well as ensuring that services meet standards of care. They also provide guidance, supervision and mentorship to youth who reside at the Y.E.S. House, supporting activities and healthy skill-building while encouraging youth in their daily activities and long-term goals.
A typical day for a Youth Advisor 2 entails engaging directly with youth residing in the shelter, helping to manage after school and weekend activities such as homework, appointments and facilitating meal preparation as well as tracking, documenting and coordinating pertinent case information. A Youth Advisor 2 models and manages safe behavior and shelter expectations to shelter residents and coworkers. Most Youth Advisor 2 shifts will occur in the shelter with occasional activities and events in the community.
Summary of Essential Job Functions:
Case Management & Service Planning
- Oversee the entire process of, advocate for, and coordinate Y.E.S. and community services to meet client needs
- Identify, organize and schedule detailed action plans for clients, utilizing specific services to move toward client goals and desired outcomes
- Manage and facilitate monthly Individualized Services Planning (ISP) meetings, ensuring including of youth support team
- Process referrals to other agencies for mental health, medical needs, and schooling
- Provide care coordination, ensuring contact with community partners and parents/guardians
- Ensure follow-through on appointments and service goals
- Identify challenges or obstacles that arise such as service gaps, duplication of services, and engage in strategic solutions
Additional Resident Engagement and Services
- Build rapport and engage with residents
- Promote community engagement, independent living skills, and healthy problem-solving abilities
- Actively plan, facilitate and engage residents in enrichment activities such as art projects, gardening, cooking, and recreation
- Guide and aid residents in working toward their established goals
- Accompany residents to appointments and community activities
- Serve as the lead point of contact to facilitate, organize and initiate extended outings, extracurricular and out of house activities
- Embody ACT Positive Youth Development principles and support residents with aspects of the ACT Self-Management framework
- Conduct shelter intake process as needed and assist with transition planning for youth exiting the shelter
- Adhere to shelter and Y.E.S. policies and monitor house expectations to ensure a safe and inclusive environment
- Provide general housekeeping including cleaning, sanitizing, cooking, and organizing
- Caseload duties, as assigned by the Shelter Manager
- Celebrate resident success and address necessary changes
- Willingness to be scheduled in the On-Call rotation
Documentation, Collaboration and Follow Up
- Document shelter activities and youth services in shelter Daily Log
- Provide appropriate documentation for resident medication in Medication Log
- In charge of Activity Planning, including completion of Activity Plan worksheet
- Inquire with Program Manager to coordinate supports for upcoming resident appointments, activities or transportation needs
- Train and support new Youth Advisors in shelter, as well as applicable interns and volunteers
- Ensures shelter tasks, such as chore lists, are reviewed, completed as needed and workload is shared
- Initiates coordination of shift lunches and breaks
- Complete all Y.E.S. specific trainings, certifications, and attend required meetings
- Collaborate with coworkers, supervisor, and community partners
- Staff at least 1 Y.E.S. outreach event annually, as coordinated with the Associate Director
Other
- Assist with planning, group management, and staffing for life skills classes
- Maintain compliance with Bridges to Health reporting and documentation
- Uphold confidentiality of information
- Participate in community partner meetings as assigned/appropriate
- Available for On Call Rotation and fill in shelter shifts as needed
- Other duties and special projects as assigned
Competency and Skill Requirements:

- Moderate skills in Microsoft Office and Google applications
- Ability to confidently represent the organization with internal and external stakeholders
- Ability to travel is required, valid driver’s license and insured vehicle
Required Training, Education, and Experience:
- First Aid/CPR Training, Food Handlers License
- Community Health Worker (CHW) certification in the State of Oregon – Current or Acquired within 6 months of position
- 2 years of experience working with youth facing multiple barriers to success
- Experience working with underserved individuals and communities
- Associate’s Degree is preferred
- Bilingual in English and Spanish preferred
