Program Summary

Wraparound with Intensive Services (WISe) is a new approach to helping Medicaid-eligible children, youth, and their families with intensive behavioral and mental health care. The program's goal is to help youth thrive in their natural supports such as homes and communities and assist in helping them meet the goals they have identified. Services are available in home and community settings and offer a system of care based on the individualized need of the child or youth.

Job Summary

The WISe Care Coordinator provides comprehensive support services assisting children, youth, and families in coping with complex behavior and mental health needs. In a team framework, the Care Coordinator works in close collaboration with a Mental Health Therapist, Parent Partner Counselor, and Youth Peer Counselor to identify goals, assess needs, and coordinate access to community resources. In addition, the Care Coordinator will conduct outreach to clients and families in their homes, school, and community.

The Care Coordinator is to guide, develop, implement, and monitor individual case plans, and Child Family Team (CFT) ensures client and family voice. This role will guide their team through the WISe process and phases and hold the central point of communication between the WISe team and the client. Services will be provided almost exclusively in the community in families' homes, foster homes, schools, etc. The primary role is to ensure that all parties work together toward stability for the child, youth, and family, including coordinating services and planning for children, youth, and families referred and screened into the WISe program.

Reporting to the Director of Community-Based Services, the Care Coordinator is responsible for overseeing all aspects of the WISe program and will carry out the responsibilities of the position as defined below:

Primary Job Functions

  • Build, plan, facilitate, and communicate Child and Family Team (CFT) meeting details with WISe team members, clients, and families
  • Maintain a caseload of 15 clients
  • Complete CANS training and assessment.
  • Ensure to incorporate input and needs of the youth/parent when forming the team
  • Establish and sustain an influential team culture by inviting CFT members to propose, discuss, and accept ground rules for working together
  • Ultimately, work to transition facilitation to allow youth and/or family members to facilitate their meetings.
  • Cultivate and maintain relationships with community members and providers
  • Assist in case planning and consultation with team members on casework issues and Social Determinants of Health (SDoH) as appropriate.
  • Maintain team focus on the scope of work for the progress/movement toward acknowledgment and celebrate successes and transitions.
  • Maintain all up-to-date Cross System Care Plans for each client and any other related documentation
  • Participate in the crisis response system
  • Participate in individual and/or group supervision
  • Attend all WISe required training as defined by the County
  • Commit to and engage in self-evaluation and organizational goals, strategies, and systemic practices regarding diversity, equity, inclusion, and belonging.
  • Share in on-call rotation for after-hours/weekends response to WISe program families.
  • Connect families with community resources
  • Other work-related duties as assigned.

Knowledge & Skills

  • Ability to work effectively in cross-cultural situations
  • Strong ability to work with a diverse Eastside community, including race, ethnicity, gender, LGBTQ , religious, socio-economic status, etc.
  • Experience working with vulnerable children, youth, and families
  • Strong verbal, written communication, and interpersonal skills
  • Customer service mindset
  • Aptitude to work in a fast-paced environment, prioritize workload, complete assigned tasks, and maintain poise under pressure
  • Dependable, receptive to change, willing to learn, cooperative approach to problem-solving, flexible team player, etc.
  • Effectively work in both a team environment and independently.
  • Computer literate and able to use Electronic Health Records (EMR), telehealth, and other software programs with basic training.

Qualifications

  • Bachelor's degree preferred in the social service related-field
  • Two or more years’ experience in case management working within community organizations and home visiting.
  • Clearance fingerprints and any other state or federal requirements
  • Own vehicle; valid Washington Driver's License w/ proof of auto insurance
  • Ability to work as part of and supervise a multidisciplinary team
  • Robust organizational, coordination, writing, verbal, and interpersonal skills
  • Bilingual and bicultural skills are a plus!
  • Experience in WISe or other WRAP programs is a plus!
  • Full-time (40 hours/week)
  • Must maintain a flexible schedule to work some evenings and weekends
  • Travel throughout Eastside King County
  • Creativity and flexibility are essential in assuming significant responsibility
  • Experience working in racially, ethnically, and socioeconomically diverse urban communities is preferred.

Physical / Working Requirements

  • Work in general office and home-visiting conditions
  • Travel throughout Eastside King County w/ mileage reimbursement
  • Frequent sitting, standing, walking, and driving (HV)
  • Ability to use hands and handle objects
  • Adequate vision and hearing is necessary for duties
  • Occasional pushing/pulling/lifting/carrying up to 25 pounds
  • Speaking/talking on the telephone, virtual, and in-person
  • Ability to travel and work in multiple settings
  • Ability to work evening hours and adjust work schedule to meet business needs.
