What are the responsibilities and job description for the Nurse Home Visitor position at Youth Development, Inc.?
NURSE HOME VISITOR: The Nurse-Family Partnership (NFP) nurse home visitor is responsible for providing comprehensive nursing services to women and their families who meet eligibility for the Nurse-Family Partnership Program (NFP Program). The Nurse Home Visitor is responsible for maintaining the highest level of standards in clinical nursing practice and adherence to the NFP model, NFP Visit-to-Visit Guidelines and standards of NFP and YDI policies and procedures.
MINIMUM QUALIFICATIONS:
Associate degree or bachelor’s degree in nursing. Two (2) years’ experience in maternal/child, public health, home visitation or mental health/behavioral nursing preferred.
Must possess current RN license (in good standing)
Experience serving parents and children experiencing concentrated and overlapping barriers to accessing the care they need due to social, economic, and environmental disadvantages.
Excellent written and verbal communication skills.
Must have computer skills, including proficiency in data entry and documentation within data management systems or databases.
Must be able to work flexible work schedules including some weekends and evenings.
Must complete all required NFP initial education.
JOB CHARACTERISTICS/PHYSICAL DEMANDS:
This position is exempt from overtime provisions of the Fair Labor Standards Act. This is a professional position which requires excellent organizational and communication skills, as well as judgment, tact and diplomacy in contacts with program clients, employees, and community representatives.
Strict adherence is required to all regulations concerning confidentiality.
Background investigation to include fingerprinting will be required.
Physical is required upon hire and may be required intermittently.
Frequent travel is required, including Bernalillo, Torrance, Sandoval, Valencia, and Rio Arriba counties. Valid NM driver’s license and proof of satisfactory car insurance required.
