Position Overview

Provides intensive case management services for youth ages 6 – 17 who are chronically truant, ungovernable, and/or runaways, are court involved or are likely to enter the CINS (Child-In-Need-of-Services) petition process, and may require more intensive and lengthy services. Services may be court ordered or referred through Case Staffing Committee. Provide case management services to CINS/FINS youth that may be receiving therapeutic services in YCC’s residential crisis care program or Family Link nonresidential services with little success. Services will be provided during traditional and non-traditional business hours in the 5-county area to include community-based care, homes, and/or schools and connect youth and families to a coordinated, comprehensive array of services for ongoing needs. The goal is to prevent youth from going into the DJJ or DCF system.

Knowledge:

  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation.
  • Knowledge of CINS/FINS process
  • Knowledge of strengths-based programming, positive youth development and trauma-informed care service models.
  • Knowledge of social service ethics.

Skills:

  • Skillfully provide full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
  • Be aware of others' reactions and understanding why they react as they do.
  • Use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
  • Actively look for ways to help youth and families who are physically and emotionally challenging and, in most cases, the most difficult youth at the agency.

Abilities:

  • The ability to remain calm and professional during stressful and challenging situations.
  • The ability to engage youth and their families when all parties are not willing.
  • The ability to initiate change agent activities which will promote positive outcomes.

Position Duties and Responsibilities

  • Perform needs assessments and additional assessments/screenings as required through interviews and observation and develop measurable goal plans based on experience and knowledge with the client/family’s involvement. Submit all required data collection instruments to the designated YCC staff in a complete and timely manner. Prepare and maintain all required case management records and assessments, while maintaining confidentiality.
  • Meet with clients and families to address concerns presented during the intake session. Such concerns may pertain to overcoming peer stressors or traumatic life experiences, adjusting to life circumstances, and not attending school with the overall goal of developing positive coping skills in order to remain in school and out of any system. Coordinate services to assist the client/family with these concerns. Work in conjunction with therapist assigned to case to provide collateral information necessary for positive outcomes of case
  • Perform targeted outreach services to the community partners in YCC’s five (5) county service area. Conduct home and school-visits for clients to document progress.
  • Directly provides or arranges for and coordinates medical and dental services, substance abuse education and treatment, school services, transportation (arranging/coordinating) and other family support services.
  • Participate in the Case Staffing Committee and assist in preparing CINS Court documentation as required.
  • Responsible for completion of training objectives within established timeframes.
  • All other duties assigned.

Education and Experience

Education and Experience: Bachelor’s degree in social work, sociology, psychology, or human services-related field from an accredited college or university recognized by Florida Department of Education; one (1) year of related experience. A valid Florida Driver’s License and safe driving record is required.

Mathematical Skills and Reasoning Ability

Basic and/or advanced computational skills may be necessary.

Physical Demands

This is a sedentary position, and the incumbent will be required to sit approximately 70 per cent of the time. Little to moderate lifting of less than 25 lbs. may be required.

Work Environment

While performing the duties of this job, the employee is occasionally exposed to outside weather conditions.

The noise level in the work environment is usually moderate.
