What are the responsibilities and job description for the Transitional Housing Case Worker position at Youth and Family Programs?
Youth and Family Programs is seeking to hire a Transitional Housing Case Manager for our Chico location.
Responsibilities:
Will be responsible for carrying cases and coordinating services for children and their families. This includes working with biological families, foster families and county social workers. Having regular contact with clients, maintaining case notes, assisting foster parents with needs specific to clients, monitor or supervise visits as needed. Attend family centered team meetings (FCT, CFT, TPM, FARE, FPM, etc...) and any additional County or school meetings as needed or required. Knows and follows confidentiality law and procedures at all times. Completes SAFE Home Studies and re-certifications.
Is on-call as needed.
Requirements:
Must have a bachelors degree.
Acceptable degrees: Social Work or Social Welfare; Marriage, Family and Child Counseling; Child Psychology; Child Development; Counseling Psychology; or Social Psychology.
