We are seeking qualified Speech Therapist to service our patients though out the Grandstrand area. The Speech Therapist is responsible for evaluating and treating patients with speech, language, cognitive-communication, and swallowing disorders. This role supports patients across various care settingshome-based, facility-based, and outpatientand works closely with interdisciplinary care teams. The Speech Therapist will contribute to reducing hospitalizations and enhancing patient quality of life through person-centered therapy and proactive monitoring.
Our therapist conduct patient in-person visits in various settings including homes and facilities. This is a full-time salary-based position, with the opportunity to receive a monthly bonus and vehicle allowance.
The following shift(s) are available:
- 8-hour shift (Monday-Friday; 8:00am-5:00pm)

What Are We Looking For?
Your Health is currently looking for a Clinical Coordinator to join our growing primary care family. A successful Speech Therapist will be able to perform these essential duties and responsibilities. Areas of Responsibility:
- Conduct comprehensive assessments for speech, language, cognition, and swallowing.
- Develop and implement individualized treatment plans based on patient needs.
- Perform re-evaluations and modify treatment approaches as appropriate.
- Monitor and document patient progress toward goals.
- Collaborate with physicians, nurse practitioners, occupational therapists, physical therapists,
and care coordinators. - Communicate effectively with interdisciplinary teams to align therapy goals with care plans.
- Educate patients, families, and caregivers on communication strategies and swallowing
precautions. - Facilitate daily visits with providers (MD, NP, PA) to ensure timely communication,
coordination of care, and alignment on patient treatment plans. - Serve as a liaison between therapy services and providers by sharing updates, raising
concerns, and reinforcing care plan goals. - Integrate provider input into therapy interventions and document appropriately to
maintain continuity of care. - Maintain accurate and timely clinical documentation per regulatory and company standards.
- Ensure compliance with HIPAA, licensure regulations, and billing guidelines.
- Participate in quality assurance initiatives and audits as required.
- Provide training to caregivers on speech exercises, assistive communication tools, and home
management strategies. - Support transitions of care by preparing discharge plans and community referrals when
appropriate.
Qualifications:
- Masters degree in Speech-Language Pathology from an accredited program.
- Active state license and ASHA Certification (CCC-SLP).
- Minimum of 12 years of experience in adult or geriatric therapy preferred.
- Experience with neurological and cognitive impairments is a plus
- Strong clinical documentation, communication, and time management skills.
- Willingness to travel across assigned regions as needed.
- Must be proficient in utilizing technology tools and platforms for remote patient monitoring,
telemedicine, and electronic medical records. - Ability to manage and demonstrate effective leadership skills.
- Should demonstrate good interpersonal and communication skills under all conditions and
circumstances. - Ability to foster a cooperative work environment.
- Team player with ability to manage multiple responsibilities and demonstrate sound
judgment. - Must be able to work flexible hours and travel between offices, facilities, etc. Must be a
licensed driver with an automobile that is insured in accordance with state and/or
organizational requirements and is in good working order.
