Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately
- manage medical documentation in compliance with HIPAA regulations and healthcare standards
- Support clients with memory care needs, including Alzheimer's and dementia care, ensuring safety and engagement
- Perform patient observation to identify changes in condition or behavior, communicating findings promptly to healthcare professionals
- Provide companionship and emotional support to enhance mental well-being during home or community visits
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry and meal preparation
Requirements
- Valid certification as a Home Health Aide (HHA) or Certified Home Health Aide (CHHA) from an accredited program. Company can provide training too
- Proven experience working with individuals with developmental disabilities, dementia, or in hospice care is highly desirable
- Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate medical documentation
- Knowledge of medical terminology relevant to patient care and medication administration procedures
- CPR certification and training in catheter care, hospice support, and the use of mobility aids like Hoyer lifts
- Experience in long-term care settings such as nursing homes or assisted living facilities is preferred
- Ability to work independently in home and community environments while adhering to safety protocols
