ORGANIZATION SUMMARY:
Young Parents United, Inc. is a 501c3 nonprofit organization providing support services, advocacy and resource coordination designed to meet the unique needs of teen parents and their children, break destructive cycles and open the doors for growth and opportunities to achieve these successful outcomes for struggling teen parents. The organization fulfills its mission to help change the landscape for very young parents -- through a combination of case management, collaboration with other agencies, mentoring, child development and parenting skills training, educational programs, assistance with housing stability, high school education completion, college planning, employment assistance, homework help, childcare planning, a safety network, social connections, family activities and much more.
CASE MANAGER RESPONSIBILITIES:
- Accomplishes clients’ care coordination by assessing client needs; developing, monitoring, and evaluating Service plans and progress; facilitating interdisciplinary approaches; monitoring performance.
- Enrolls new clients by reviewing records and applications; conducting orientations.
- Determines clients’ requirements by completing intake interviews; determining need for any emergent, physical, safety, educational, therapeutic medical, psycho-social, or other needs
- Establishes client objectives and plan, sets schedules and routines; coordinates services being provided; arranges resources, including transportation and escort.
- Monitors cases by verifying clients’ attendance; observing and evaluating activities, engagement, programs, and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crisis; providing ongoing support.
- Maintains clients’ records by documenting daily, reviewing case notes; logging events and progress.
- Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying barriers to progress influenced by client engagement and unavoidable circumstances.
- Prepares clients’ discharge from the program, coordinating discharge and post-discharge goals / transition plan, assisting family members and providing resources.
- Supports clients by providing educational resources; balancing program requirements with learning opportunities; evaluating the application of learning to changes in outcomes.
- Improves client progress results by studying, evaluating, and redesigning processes; implementing changes; rewriting service plans for clients as needed.
- Prepares reports by collecting, analyzing, and summarizing client outcomes and results data and trends; compiling statistics
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; and participating in professional organizations.
- Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

QUALIFICATIONS:
- 2 years in human services, possess demonstrated commitment to individuals and/or families in need
- Bachelor’s Degree or above preferred
- Professional and/or personal experience supporting individuals with Hx of trauma, mental health challenges, poverty or challenging behaviors highly preferred
- Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than 2 points within the last 3 years
