Head Start Health and Nutrition Coordinator

POSITION SUMMARY

The Health & Nutrition Coordinator works in collaboration with the Operations Executive of Early Education and Executive Director of Early Education to plan and coordinate health, dental, immunizations, and nutrition services to children and families enrolled in early learning programs. The Health & Nutrition Coordinator provides health support by on-going monitoring of children's health documentation, observing children to assess needs, conducting health screenings, implementing specialized health procedures, creating food menus, and including food accommodations. The Health & Nutrition Coordinator also serves as the Administrator of the California Adult & Child Food Program (CACFP).  The Health and Nutrition Coordinator collaborates and consults with Registered Nurse and Dietician consultants to ensure that health and nutritional needs of children in the program are met.

ESSENTIAL FUNCTIONS

Health Services

  • Work in consultation with a Registered Nurse to review and evaluate student health records and assess medical needs based on medical histories, chronic conditions, and special health needs to assure students receive all required screenings including timely follow-up; review student data to determine need for counseling or medical intervention; obtain medical records as needed.
  • Conduct health care screenings within established timelines including, but not limited to, hearing, vision, and growth measurements.
  • Record health data related to such screenings.
  • Communicate screening results and provide parent education related to results.
  • Implement other procedures to meet individualized needs as prescribed by the child’s health care provider.
  • Inform teachers of physical or health concerns of students.
  • Follow-up with parents regarding health concerns, medical documents needed, or school exclusions due to health and other as needed information.
  • Work with Family Service Specialists, Site Directors and other key team members to ensure completeness and accuracy of student files and health forms; reviews and reconciles student health records with staff and RN.
  • Coordinate ordering health supplies.
  • Conduct health education workshops for staff, parents and students regarding safety, health procedures, child abuse prevention, communicable diseases, first aid, medical instructions for medications, procedures and information on diseases/disorders annually and as needed.
  • Prepare health related reports, correspondence and notices and updates the annual Health Services policies and procedures for providing, monitoring, and evaluating health care services.
  • Complete the CA Annual Shots for Schools immunization report.
  • Develop and maintain community resource networks and coordinate with Executive Director of Early Education to ensure the program maintains community resource networks and negotiates interagency agreements and contracts with local providers.
  • Participate in multi-disciplinary team meetings for intervention and follow-up purposes for children identified with individualized health needs.
  • Utilize data management systems to track and monitor cases and the effectiveness of service delivery.
  • Participate in agency self-assessment and program planning to build community-based networks and child development and health partnerships based on program needs to best serve children and families in our community.
  • Ensure implementation of the Individualized Health Plan (IHP) based on a student’s history and medical diagnosis, in consultation with the registered nurse who is responsible for development of the IHP
  • Serve on the Health and Mental Health Services Advisory Committee
  • Attend required trainings and meetings, including those required by LACOE. 
  • Communicate and collaborate with other comprehensive service areas, including LACOE.

Nutrition Services

  • Work in consultation with the Registered Dietitian to identify and assess children’s nutritional needs
  • Work in partnership with program management and staff to meet the nutritional needs of children in the program.
  • Collaborate with the Operations Executive Director of Early Education to plan and evaluate the work of staff and consultants related to the design and delivery of nutritional services programs.
  • In collaboration with the Operations Executive Director of Early Education, evaluate and monitor how food is prepared and served; shares results with Operations Executive Director of Early Education and Executive Director of Early Learning and coordinate corrective measures, as needed.
  • Participate in planning and coordinating nutritional screenings and specialized CPAC training standards.
  • Provide support for wellness and child nutrition programs to recommend nutritional guidelines.
  • Serve as liaison between the registered dietitian, site directors, teachers, and other key program team members to ensure student dietary needs are properly met.

Child and Adult Care Food Program (CACFP)

  • Act as the administrator for the Association’s CACFP program.
  • Responsible for ensuring the needs of all participants, including food substitutions, are met through our health screening, nutrition screening, and physical exams.
  • Plans monthly menus in collaboration with the Registered Dietitian and Early Education Staff on the CACFP meal patterns and ensures that all nutritional guidelines are met.
  • Reviews the CDSS online training offers to determine which training courses support the roles of each staff member and participate in annual mandatory training courses.
  • Reviews meal counts and provides prior month’s data to the finance department in a timely manner.
  • Collaborate with Site Directors and Operations Executive of Early Education to ensure all food and substitution items are purchased and within CACFP nutritional standards.
  • Ensures each student has access to the appropriate portion of food groups as established by CACFP for their age group.
  • Responsible for ensuring that Head Start Program Performance Standards, the LACOE Health and Nutrition GIM mandates, and all CCLD requirements are incorporated into the program’s daily services
  • Track children's nutritional outcomes in all required databases and record keeping platforms to ensure compliance with grant requirements.  

Other Essential Functions

  • Participate in Safety Committee and the Collaborative Team Meetings
  • Adheres to established Standard of Conduct and ensures confidentiality and mandated reporter requirements.
  • Participate as a member of the overall YMCA Early Learning Readiness department.
  • Participate in the completion of at least 15 hours of professional development annually.
  • Travel, as required, for off-site meetings, trainings, and events.
  • Attend staff meetings and trainings as required.
  • Uphold YMCA policies for safety, supervision, mandated reporting and risk management.

Qualifications:

MINIMUM QUALIFICATIONS

  • EDUCATION: Bachelor of Science degree in Health Science, Public Health, or Registered Nurse.
  • SPECIALIZED SKILLS: Bilingual (English/Spanish), preferred. Strong oral and written communication skills; Experience with ChildPlus, preferred. Knowledge of Community Care Licensing Regulations. Capacity to read, analyze, interpret, health and nutritional data for planning, monitoring, internal and external reporting, and continuous quality improvement for children ages 0-5. Experience with Child Plus preferred; proficiency in Microsoft Office Suite required; ability to learn new technologies
  • RELATED EXPIERENCE: Three years of professional health services experience or related field involving the coordination of health and/or nutrition services. Experience with community health, occupational health, school nursing, pediatrics, public health, health education, or maternal child health preferred.
  • CERTIFICATIONS & LICENCES: Current approved EMSA Adult, Infant and Child CPR, First Aid, & AED Certification (8 hours). Completion of child abuse prevention training prior to first day in program. Completed compliance courses, as assigned. Ability to pass fingerprint clearances. Must have reliable transportation, current auto insurance, and current Class C California Driver’s License.
  • IMMUNIZATION & PHYSICAL: Current TB test clearance (must be submitted every two years). Cleared physical exam. Current Influenza (limited exceptions), pertussis, and measles vaccinations.

WORK ENVIRONMENT/MINIMUM PHYSICAL REQUIREMENTS

  • You must have the physical, visual, and auditory ability to perform the essential functions of the job with or without reasonable accommodation.
  • This position requires extended sitting, frequent computer use (hands/fingers/wrists), light lifting (under 20 lbs), occasional walking/standing/reaching/bending for files or supplies, and clear speaking/hearing for communication, all within a standard office setting with minimal physical exertion or risk.

  • Ability to conduct observations, assessments, and intervention in various classrooms, homes, and community settings related to health and nutrition.

Job is performed in indoor/outdoor environments at multiple program sites throughout the year.

Ability to lift 25-30lbs.

  • Reliable transportation, as this position requires regular travel to program sites for assessments, consultations, and crisis response. This position also requires travel to off-site locations throughout LA County for meetings, training, and other work-related activities. All work-related mileage in excess to normal daily commute is a reimbursement employee expense.
