Responsibilities
- Coordinate with interdisciplinary teams to develop and implement individualized care plans that meet patient needs, including dementia, Alzheimer's, hospice, and memory care.
- Maintain accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, including medical records, progress notes, and incident reports.
- Conduct regular patient observations to monitor health status, identify potential issues early, and communicate findings with healthcare providers.
- Ensure compliance with all regulatory requirements related to home health services and long-term care experience.
Skills
- Experience in home health or community-based caregiving environments with a focus on senior care or assisted living settings.
- Proficiency in using EMR/EHR systems for documentation and medical records management.
- Strong knowledge of medical terminology, medication administration procedures, and clinical skills like vital signs measurement and catheter care.
- Demonstrated ability to work effectively with individuals with disabilities or developmental disabilities, including dementia or Alzheimer's disease.
- Excellent communication skills for coordinating with patients, families, and healthcare teams.
- Ability to handle sensitive situations with compassion while maintaining professionalism under pressure.
Pay: $110,000.00 - $140,000.00 per year
