About Young Community Developers

Established in 1973, YCD is a nonprofit organization that serves residents of all ages living in the most historically underserved communities of San Francisco. YCD takes a holistic approach via a delivery eco-system of services designed to empower residents in achieving economic mobility, in turn, building and sustaining a positive community by providing them with education opportunities, workforce development training, and social services. Program Overview

YCD offers services to assist high at-risk and in-risk individuals, ages 14- 35, who have been involved with the justice system. We provide these customers with paid job readiness training; subsidized work experience; and employment and education placement services that align with their interests. Our intensive one-on-one support model is designed to guide customers through the re-entry process by overcoming employment barriers, providing resources to advance their job search, and attaining employment opportunities.

POSITION SUMMARY

The role of the Justice Services Care Coordinator (JSCC)/Life Coach is principally responsible for providing case management services to youth involved in San Francisco's juvenile justice system, leading connections and referrals based on assessments that link young people to additional supports, education, enrichment, and work-related opportunities that are available throughout the city. JSCCs play a critical role in supporting justice-involved youth ages 12–25 through intensive, individualized care coordination.

JSCCs are proactive in meeting youth and their families where they are-physically and developmentally-by being mobile, building on their strengths, supporting their goals, and when relevant, ensuring that youth are supported in meeting court-ordered conditions of release. The JSCC helps youth navigate complex systems—juvenile justice, education, mental health, and community resources—by building authentic relationships, developing individualized care plans, and ensuring access to supportive services that promote long-term success and stability. The Justice Services Care Coordinators are readily available for referrals, from the entry point of arrest or detention. JSCCs develop community release plans in collaboration with the defense team, create individual case plans, and ensure that youth complete intended goals and outcomes. JSCCs work with youth until they successfully complete or surpass all legal obligations determined by San Francisco Juvenile Probation Department and courts.

This role emphasizes culturally responsive, trauma-informed, and youth-centered approaches to reduce recidivism and enhance self-determination, personal growth, and community reintegration. Justice Services Care Coordinators work in coordination with Community Assessment Referral Center (CARC)to ensure that every youth who is cited and/or booked into the Juvenile Justice Center, who is not diverted to CARC, is paired with a community-based case management agency, specifically Young Community Developers (YCD) for adult guidance and systems navigation. 

 

RESPONSIBILITIES:

  • Receive referrals from courts, probation, or diversion programs
  • Conduct initial youth and family intake, including a strength-based assessment while in juvenile detention or in community
  • Visit youth while in juvenile detention prior to release
  • Advocate for alternatives to detention and support restorative and rehabilitative approaches
  • Schedule and attend probation check-ins with youth as needed probation/attorney intakes
  • Develop individualized service plans (ISPs) aligned with legal mandates, community release plans and personal goal setting
  • Coordinate service referrals for education, mental health, employment, housing, substance use support, family counseling, etc.
  • Assist youth and families in understanding legal obligations, detention release conditions, resources, and timelines
  • Act as liaison between youth, families, schools, court, judges, probation officers, and service providers
  • Collaborate with parents/caregivers to ensure a youth and family-centered support including transportation to and from court and other legal appointments
  • Provide academic resources and support toward education achievement, and linkage to family-strengthening services
  • Connect youth to community-based mentors, events, and programs for positive engagement
  • Attend all court hearings, probation, attorney intake meetings and youth centered meetings (i.e. CFT, IEP, school reentry, or transfer meetings, and/or expulsion hearings)
  • Provide consistent check-ins (frequency based on need and level of engagement required)
  • Help youth set and achieve SMART goals across key life domains (e.g., education, wellness, relationships)
  • Celebrate achievements and support youth in overcoming barriers and setbacks, including providing financial barrier support and milestones incentives for goal completion
  • Maintain accurate case notes, intake records, individual service plans, court reports, progress updates, and monthly summaries
  • Submit timely documentation to meet legal and programmatic reporting requirements
  • Participate in case conferencing with partners and referring agencies
  • Respond to urgent needs with appropriate de-escalation and referrals
  • Build relationships with school personnel, CBOs, law enforcement, and courts
  • Model restorative justice, resilience, and culturally affirming practices in all interactions

What you will bring

  • Minimum 2 years’ experience working with justice-involved youth, transition-age youth, or in a community-based case management role
  • Knowledge of the juvenile justice system, youth development principles, and trauma-informed care
  • Strong documentation and case planning skills
  • Ability to demonstrate competency with using Microsoft Office Suite, Google platform, Zoom and other technology platforms
  • Familiarity with wraparound and multidisciplinary approaches to youth services
  • Demonstrated ability to build trust and rapport with high-risk youth and families
  • Commitment to strengths-based, youth-centered, and non-punitive approaches
  • Ability to work flexible hours, including evenings and occasional weekends & holidays
  • Must possess a valid California Driver’s License and clean driving record

Preferred Qualifications

  • Lived experience, system involvement and/or deep understanding of urban youth culture
  • Bilingual (English/Spanish or other relevant languages)
  • Experience working in SF neighborhoods impacted by community violence or displacement
  • Knowledge of Bay Area community resources 
  • Familiarity with documentation systems (e.g., Cityspan, CMS, Salesforce, etc.)


ADDITIONAL QUALIFICATIONS:
To best mitigate the spread of disease and protect our staff, it is a condition of employment that all employees be fully vaccinated against COVID-19. Because YCD prioritizes the health and well-being of employees, proof of vaccination will be required upon hire and be kept on file for each employee.

COMPENSATION: $28.00-$33.00/hour
