Key/Essential Functions & Responsibilities:

  • Provides oversight and supervision of the START Therapeutic Coaches through regular check-ins, providing regular feedback, coaching and conducting discussions related to performance or other concerns.
  • Assigns cases to Therapeutic Coaches and assists with scheduling, planning and clinical plan development related to assigned caseloads, ensuring appropriate frequency of visits and coordination with Coordinator based on level of support needed, activity level and/or project scope (e.g. tier).
  • Assists Therapeutic Coaches with identifying appropriate strategies and interventions to meet the goals of the service for persons supported, develops materials for coaching sessions and provides triage of issues as needed; escalates complex cases to supervisor or other START clinical staff accordingly.
  • Monitors completion of all necessary trainings for staff across assigned region, to ensure compliance with YAI policies, timelines and other applicable rules or regulations.
  • Assists with referrals for coaching, including facilitation of admission planning meetings, screening for enrollment in START coaching services, completion of coaching intake assessments across assigned region(s) consistent with applicable timelines and/or referrals to clinical team, Medical or Clinical Director or applicable State agencies, as appropriate.
  • Collaborates with the Coordinators and Clinical Director in all aspects of planning and scheduling related to START Therapeutic Coach utilization, including assuring availability of emergency coaching services when deemed necessary by the team.
  • Regularly communicates with the caregivers and circles of support of persons receiving coaching services to monitor services and progress, providing updates regarding care and/or concerns or issues and providing education and assistance to support systems when needed.
  • Ensures that all coaching activities are appropriately documented by staff and completes and maintains all applicable documentation, paperwork and/or records on a timely basis including resolution of concerns and reporting of incidents.
  • Accurately enters and ensures Therapeutic Coaches have entered all required data into START Information Reporting System (SIRS) and eVero/Digital Agency, as well as other appropriate systems, as required by state, on a timely and contemporaneous basis.
  • Communicates with the START clinical team regarding the person’s and caregiver’s responses to therapeutic coaching to track progress and evaluate strategies to incorporate into the Cross Systems Crisis Prevention and Intervention Plan.
  • Participates in recurring consultation, staff and other meetings with START leadership, clinical team and START Therapeutic Coaching program.
  • Supports regular outreach with community partners to maintain linkages and relationships.
  • Facilitates discharge planning meetings and ensures all assessments and data are completed and collected to support successful transition out of therapeutic coaching services.
  • Serves as Administrator-on-Call (Available/On Call Supervisor) on a regular, rotating basis with other program leadership and provides coverage participating in 24/7 on-call crisis support, if needed.
  • Completes and maintains all necessary trainings based on assigned program/region to ensure compliance with YAI policies, NCSS and applicable state regulatory agency (e.g. OPWDD, DDS) requirements, including obtaining and maintaining START Coordinator certification.
  • Participates in the Center for START Services National Network (NCSS), which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference and/or other START events, as recommended.
  • Complies with all Federal, State, Local and other relevant regulatory agency requirements, including the Health Insurance Portability and Accountability Act (HIPAA).

Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g., lifting, assisting lifting, standing, etc.)

  • Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Services field; and
  • At least (two) 2 years' experience working with the I/DD population and/or behavioral health needs; and
  • Experience in therapeutic programming for people with challenging behavior including familiarity with expressive therapies (music therapy, art therapy, recreational therapy, occupational therapy, etc.); or
  • Satisfactory combination of education, experience and/or training as authorized by START/NCSS and/or appropriate State regulatory agency (e.g. OPWDD, DDS, etc.); however, all START Therapeutic Coaching Leads must have at least a Bachelor’s degree in human services discipline.
  • Successful completion of START Coordinator Certification training within 12 months of appointment, including demonstration of competencies in all associated areas.
  • Ability to provide on-call coverage for crisis services on a regularly recurring basis, as designated by START leadership in assigned region beyond regular work hours including weekdays, weekends, evenings and/or overnight on a 24/7 basis, as required.
  • Ability to travel regularly across assigned region(s) to conduct in-person coaching sessions and visits as required by caseload and to occasionally travel across other regions within state of operations and/or to other states as needed for NCSS meetings or events.
  • Ability to meet essential physical demands of position including using hands to handle, finger or feel objects, tools or controls walking; sitting, standing, bending, twisting, stooping, kneeling, crouching, pushing, pulling and reaching with hands and arms when providing supports; ability to lift, carry and transport items needed for therapeutic interventions weighing up to 25 pounds to/from coaching sessions, as needed.
  • Valid Driver's License required for California and New Jersey assignments; strongly preferred for assignments in New York, especially in areas with limited public transportation.
  • Ability to work with complex systems and an interest in the population is essential.

Preferred Qualification Requirements (desired requirements beyond MQRs above)

  • Familiarity with START program and services
  • Two (2) or more years’ experience supervising others in the implementation of direct service
  • Experience working with individuals with I/DD with cooccurring diagnoses and complex behavioral needs
  • Experience working with families and/or social service providers
  • Verbal and/or written fluency in Spanish highly preferred; fluency in other language(s) may be preferred based on assigned region(s) and caseload

Compensation:

  • Salary is up to 80000 USD
